This is an exciting opportunity to join a dynamic and forward thinking Community Stroke Team. You will be a confident assistant practitioner with neuro/stroke experience. As a key member of our therapy team you will be confident in building good therapeutic relationships with patients and their carers, and be able to contribute to the high functioning of the multidisciplinary team. You will work as an active member of the Stroke Team, assisting in the delivery of integrated rehabilitation in Oldham. Supporting qualified therapists (occupational therapists, physiotherapists and speech and language therapists), and specialist nurses in assessing and treating stroke patients following discharge from hospital and living in the community.

You will have responsibility for your own workload with appropriate support from the team.
You will be responsible for evaluating and amending treatment programmes and interventions within guidelines and competency frameworks supervised by the qualified staff.
You will often work independently in the community as a lone worker, in line with organisational lone worker policy.

**The work will take place in a variety of settings including**: patients’ own homes, other community settings and in-reach to hospital.

You will be given a dedicated induction programme and a full training and development plan will be supported by regular staff supervision and appraisal.

You will have an NVQ level 3, a formal Assistant Practitioner qualification and be practising as an Assistant Practitioner. We welcome forward thinking individuals who would like to be part of a dynamic stroke team working across the Oldham area.

Due to the nature of the post it is expected that you will have a full driving licence and will have regular use of an appropriate vehicle (as determined by the Trust)
