Working in collaboration with the multidisciplinary team the post holder will facilitate and develop practice related to discharge that promotes safe and timely discharge of patients while ensuring that the patient and carers experience is good.

**The post holder’s key relationships will be with**: Internal: Multi-Disciplinary Team, Discharge Hub, wards. External: Secondary and primary care teams; community nurses; social workers; networking groups

University Hospitals Sussex Foundation trust (UHSx east) is the regional teaching hospital working across two sites: the Royal Sussex County Hospital in Brighton and Princess Royal Hospital, Haywards Heath.

This is an exciting opportunity to be part of an Integrated Discharge Team (IDT) at the Princess Royal Hospital (PRH) based in Haywards heath.

The role will help the flow of patients through the hospital but also into the community. It will include;
- Liaising with patients, families and carers
- Liaising with nursing staff and community services
- Completing referral forms for the three CCG areas (Brighton and Hove, East Sussex and West Sussex)
- Answering telephone calls and directing these to the most appropriate staff
- Setting up/ attending meetings as directed
- Using IT skills to file and record documentation
- Completing Fast Track referrals and funding forms

The post would include being part of the rostered seven day working Mon - Sunday, to support discharges using various services available.

1. Ensure that grievances, disputes and complaints are resolved speedily and at the lowest possible level of the organisation.
2. Required to use own initiative, take responsibility for decision-making and prioritise own workload within a team context.
3. Required to bring to the attention of appropriate managers/supervisors where workloads/pressures become such that an appropriate level of safe and effective patient care cannot be maintained, before the situation becomes untenable.
4. Able to provide advice, demonstrate own activities or workplace routines to visitors to the department such as student nurses.
5. Required to work within appropriate Clinical Guidance Documents (CGDs).
6. To be able to assess, implement and evaluate the discharge needs of patients and or carers, and to refer for the provision of these care needs to appropriate members of the multidisciplinary team
7. Ability to act as discharge coordinator lead in a defined clinical care area to ensure safe and high quality discharge practice is undertaken.
8. Have knowledge of internal and external service teams and organisation that can support aid in the discharge of patients and initiate referrals when appropriate.
9. To supervise and train junior nursing staff to aid in their development of good discharge practice.
