Health Care Assistant

We are currently looking for a full time Health Care Assistant who will be working as a member of the St Ives District Nursing Community Team undertaking delegated duties in order to meet the needs of the needs of the service users.
You will be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users.
This is a great opportunity for an experienced, motivated and enthusiastic individual to join our service providing care in patient’s own homes.

As a Community Health care Assistant you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. We are looking for caring, responsive and diverse people who share our passion to deliver our service.
**We are looking for someone with**:
- Experience of working in the community.
- Experience of working within health or social care.
- Experience of caring for and supporting adults and older people
- Demonstrate a commitment to high quality care & service provision.
- Good communication skills.
- The ability to work flexibly and as part of a team, as well as individually.

- Make judgements on patients response to prescribed treatment/care plans, and report findings to a qualified professional in line with competency framework.
- Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
- Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.
- Promote patient independence and self-management Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body's guidelines.
- To maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne or RIO within 24hours).
- Plan and prioritise own visits to delegated patients, (and ensure these are documented on SystmOne) Ensuring patients personal care is provided, as appropriate.
