This Clinical Triage team is all about getting patients the right care at the right time in the right place.

Following an internal restructure we are launching a new model of Clinical Triage for our Herts Community Nursing teams. A new team is being created based in Hemel Hempstead, to undertake referral screening and initial remote assessment of all referrals to the service. The team will cover 7 days a week 8am to 6pm and we have a range of full time and part time positions available.

We are looking for Band 6 Community Nurses and/or Paramedics to take on this important leadership role. You will have sound clinical skills and experience in order to be able to effectively complete remote clinical assessment and referral triage and support others to do the same. You will have the organisation and communication skills to be able to work as the shift leader.

An understanding of community nursing and previous experience of working in a community nursing or similar health or social care setting in a clinical role is essential. Use of telephone and video consultation, confidence with IT, and familiarity with electronic clinical record systems would be an advantage.
1. To set and maintain standards of care in clinical triage and deploy, co-ordinate and supervise the triage team across the service to maintain effective and timely response to new referrals.
2. To provide leadership and management to a team of community nurses and support staff covering the triage function across Herts Valleys.
3. To be responsible for the team’s standards of triage, remote assessment, care planning, implementation and evaluation of individualised nursing care to patients living in the community.
4. To contribute to a rota to provide robust cover 7 days a week, 8-6pm
5. To be responsible for the team’s maintenance of patient and other records
6. To participate in the development of protocols and pathways to support self management, standardisation of clinical practice, and efficient use of resources.
7. To deputise for the Triage Team lead as needed, including acting in the role of shift leader for the triage team.

1. To demonstrate discretion and respect when communicating with patients, colleagues and other agencies.
2. To use tact and diplomacy to provide and communicate complex, sensitive information where motivational negotiating empathetic and reassurance skills are needed, where barriers exist to understanding, maintaining confidentiality within Trust Policies.
3. To communicate effectively with all members of health and social care teams. Develop networks for effective communication between health, statutory and voluntary services including patient forums.
4. To communicate and work together with other teams within Adult Community Services, and other providers, to ensure optimum patient centred care. Use various methods of communication including oral, written, electronic or other verbal or nonverbal methods.
5. To involve patients and carers in the planning of care programmes and encourage self-management where appropriate.
6. To maintain accurate nursing records, including electronic records, within the locality team.
7. To ensure all patients and their relatives cultural wishes are respected and their special mental and physical needs are met.
8. To have responsibility for conflict resolution both within and without the team.
9. To assist with marketing the District/Community Nursing Service to develop greater understanding of service delivery within the wider population.
