**JOB SUMMARY**

Patient Care Co-ordinators with an adult and child safeguarding focus will have a key role in supporting Safeguarding Lead GPs to protect our most vulnerable patients.

They will work within West Somerset PCN to provide central co-ordination for safeguarding information and communication to support care planning. The role will have the core responsibility of maintaining consistent protocols regarding safeguarding information across the practice and network. They will oversee safeguarding administration, document handling, record management and communication with patients and partner organisations.

They will also be working with the PCN team to tackle health inequalities and access to healthcare across the West Somerset population, assisting with data and contacting patients for initiatives such as immunisation and vaccination access, uptake of cancer screening and support for patients eligible for health checks.

This role is an integral part of the PCN’s multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN.

Please note that the role of a care coordinator is not a clinical role.

**Responsibilities**

**Administrative safeguarding duties**
- To oversee the workflow of safeguarding reports and documents and ensure they are circulated to appropriate clinicians in line with practice safeguarding policies.
- To summarise and code documents into patient records, highlighting anything specific to the relevant named GP in line with practice policies.
- To maintain a register of children on Child Protection Plans, Child in Need plans and those children deemed as cause for concern.
- To ensure the correct coding and alerts are added to patient records to aid clinicians in identifying those children and adults who are cause for concern.
- To liaise with patients, clinical colleagues and other stakeholders to ensure that the most up to date contact, demographic, and household details are held on record.

**Partner Communications**
- To respond to child protection meetings as advised by practice safeguarding lead, preparing appropriate documentation and sending apologies as necessary.
- To monitor DNAs for children in regard to missed clinical appointments and support clinical colleagues (e.g., Practice Nurse, usual GP, Safeguarding Lead) to follow up with patients in line with practice safeguarding policies.
- To follow up on those children who have not had their routine immunisation, as identified by administration with the appropriate clinician (e.g., Practice Nurse) in line with practice safeguarding policies.
- To support in co-ordinating communication and meetings with third parties e.g., Health Visitors, Safeguarding Leads and Social Workers. This includes liaising with all stakeholders, meeting scheduling, document distribution, taking minutes and updating the patient record in accordance with agreed actions.

**Expert Practice and Practice Development**
- Be a role model for all clinical staff by practicing safeguarding to the highest standard, whilst exercising a high degree of personal and professional autonomy.
- Be aware of local resources and disseminate updates to colleagues.
- To carry out case reviews and disseminate learning to colleagues across the PCN.
- Demonstrate commitment to developing own expertise to support robust systems, policies, professional guidance, and training so that staff across the PCN can identify and contribute to safeguarding issues.

**Administrative Vaccination and Immunisation duties**
- Proactively ensure that eligible patients receive their immunisations and vaccinations in a timely fashion.
- Ensure eligible patients are contacted through appropriate means of communication to ensure good uptake and attendance. Help patients to manage their needs through answering queries and ensuring that people have good quality written or verbal information to help them make choices about their care.
- Assist and potentially lead on the digital appointment booking system for vaccinations.
- Carry out searches on clinical systems to identify eligible patients.
- Invite eligible cohorts through appropriate means i.e., text / call / letter on a regular basis.
- Follow up DNA appointments of immunisations.
- Promote immunisations to eligible patients to encourage uptake.
- Working with other members of the PCN team, seek to innovate vaccination delivery for hard-to-reach groups and monitor uptake across the PCN for specific groups.

**Other Duties**
- Support the practices in your PCN to improving local uptake of National Cancer Screening Programmes, understand the practices data for uptake on screening and understand how the practice can carry out the screening.
