The purpose of this job description is to outline the level of responsibility and accountability of this post. This will ensure that all work undertaken by our staff is clearly identified and carried out under clear lines of accountability. The Specialist Practitioner for the Youth Justice Service will work with children and young people aged 10-18 years old open to the Youth Justice Service (YJS) and hold a Clinical caseload for this cohort. The post-holder will have overall responsibility for the day-to-day management of a defined caseload of children who live in Tower Hamlets and City of London and are allocated in the Youth Justice Service.

The post holder will be based primarily with the YJS and attached to a multi-disciplinary team delivering an outcome focused care /support packages for children and young people who present with a variety of issues including mental /emotional well-being as well as physical health concerns and behavioural difficulties who have traditionally found it difficult to access services. He/she will be responsible for effectively aligning Health management plans, and roles in line with management of long-term conditions and medical care plans in line with local and national guidance/pathways. This will be achieved by means of identification, assessment, and evaluation of holistic health needs, and developing/delivering Public Health strategies in partnership with identified YJS Case Manager or Case Prevention Officer as well as multi-agency partners. The delivery focus will be aligned to the Health Spot model of care within Tower Hamlets.

He/she will work collaboratively with the NEET (Not in Education, Employment or Training) programme in supporting young people to optimise uptake and access of services that will support their management of health and well-being. Key Responsibilities: To undertake a holistic health and wellbeing of all children allocated in the Youth Justice Service and receive and make referrals from and to professionals working within the field of specialist practice. To Identify, prioritise, develop, and implement programmes of care to meet individual and local needs, considering cultural diversity. This will be achieved through evidence-based assessments, effective care planning and implementation of a range of support packages for individual children, utilising best available evidence.

To be the main health representative for a defined client group of children, in assessing and meeting their identified health needs through an ongoing programme of health surveillance, intervention, monitoring, and referral. To lead on and co-ordinate community-based specialist clinics in partnership with integrated Health Spot team within defined area/s of practice e.g., Audiology, Enuresis, Sexual Health, Complex Care management. To attend relevant Youth Justice Service panels (e.g. risk management & resource panel and out-of-court disposal panels) pertaining to individual children within the caseload or to represent a colleague/s as appropriate within the Child and Family Health Team.

To be a Health lead in the facilitation of health plans for individual children accessing the YJS. To support this process there will be positive relationships with the Youth Services/Home Schooled /PRUs/Colleges /Mainstream schools and Universal /specialist services to enhance social inclusion in all plans. To develop pathways in liaison with the key stakeholders that will support transition to mainstream services. To ensure effective joint working between agencies and professionals and effective information sharing to safeguarding children and young people.

To act as case coordinator for an agreed caseload and fully utilise routine outcome measures in collaboration with service user and families. To liaise with other professionals, offer consultation and joint working to provide early interventions in and prevention of mental health problems in children/Young people. To develop an effective interface between the GP and partnership agencies. To work in partnership with parents, children / young people, schools, statutory and voluntary agencies and other healthcare professionals, in identifying vulnerable children and children with specific medical conditions, in line with The THGPCG Policies and Procedures.

To act as the health advisor in setting up / reviewing of individual protocols / care plans for children with specific medical needs and to provide health information and training to all relevant staff. To work in partnership with social services and all other agencies involved with child protection and safeguarding issues, in accordance to Working Together, The London Child Protection procedures and The THGPCGs Safeguarding procedures and policies. To represent health as an agency at Child Protection conferences, network meetings, planning meetings etc., for children and young people aged 0-19 and up to 25 with SEND.
