Discharge Coordinator

**Job description**
As a discharge coordinator with Stayathome Cornwall you would be required to:
- Work closely with the hospital and attend daily meetings to assist with facilitating the effective flow and discharge of patients from hospital into our community teams.
- Responsible for organising holistic patient assessments and coordinating our service in line with discharge times.
- Attend MDT (Multidisciplinary Team) meetings.
- Liaise with hospitals and medical professionals
**About You**
- Good IT skills
- Good organisational skills
- Full, clean driving license and own transport
- Great communication skills
