**Location of work**
Three Chequers Practice:
- Three Swans Surgery, Rollestone Street, Salisbury.
- Endless Street Surgery, 72 Endless Street, Salisbury.
**Contracted days**
Monday, Tuesday and Friday, 6-8pm available. Please state preference.
**Hours**
Part-time hours.
**Salary**
£10.42 - £11.16 per hour, depending on experience. NHS pension available.

**Main purpose of the role**
Provide care to patients who wish to be seen by a health care assistant within the Hub environment providing high quality evidence based nursing services to the Mid Hampshire Practice population. Patients can be pre-booked into a routine primary care appointment at the GP Hub by any GP Practice within our locality. The GP Hub acts as an extension to usual GP Practice services offering appointments at times more convenient to some patients.
- To provide treatments, screening and health education services working strictly in accordance with specific MHH protocols and guidelines.
- To ensure clear channels of communication exist between clinical and administrative teams.
- To project a positive and friendly image to our patients and other professionals.
**Key task and responsibilities**
**Providing screening, treatment services and health education advice**:
- ECG, Blood Pressure, pulse oximetry and pulse rate and rhythm
- Height and weight (BMI)
- Ear Irrigation
- Spirometry
- Wound care, dressing, removal of sutures / clips
- Leg Ulcer Management including Doppler and Compression
- Health screening, NHS Health Checks (to include smoking, alcohol and lifestyle)
- Provide general and specific health screenings to the Hub patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary
- Ensure the cleanliness of any clinical equipment used
This list is not exhaustive. Full training will be provided should additional services be required within this role.
**Administration and Professional Responsibilities**:
- Ensure accurate notes of all consultations and treatments are recorded in the patient’s records
- Attend and participate in Hub meetings as required
- Maintain continued education by attendance at courses and study days offered by MHH or external providers that are relevant or necessary for personal/professional development
- Restocking, ordering and maintenance of stock, clinical areas and consulting rooms
- Demonstrate due regard for safeguarding and promoting the welfare of children and adults.
- Ensure auditing of clinical and emergency equipment is maintained according to MHH policies and protocols.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. The Health care assistant will ensure confidential information (written/oral or electronic) is preserved at all times whether at or away from work
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, hub staff and other healthcare workers. All such information from any source is to be regarded as strictly confidential
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the hub may only be divulged to authorised persons in accordance with the organisations policies and procedures relating to confidentiality and the protection of personal and sensitive data
