We are developing our successful Home Improvement Agency delivery in the borough and are looking to appoint a pro-active Liaison Officer to help further expand our Healthy Homes offer. The ideal person for this role will be responsible for:

- Developing and delivering our Healthy Homes project, ensuring the service is promoted and delivered borough wide.
- Supporting local homeowners to live safe and healthy lives in their homes.

**RESPONSIBLE TO**:Care Network Operations and Services Manager

**ROLE**:To take a lead role in the provision of a comprehensive Healthy Homes advice and assistance service

To develop and deliver Healthy Homes projects ensuring the service is promoted and delivered boroughwide.

To support homeowners to live safe and healthy lives in their homes.

To ensure that the service is delivered to an excellent standard to meet Care

Network’s strategic aims and the contractual requirements of our funding partners and the aspirations of our clients.

**SALARY**: £23,569 per annum plus a 10% employers pension contribution

**HOURS**:Full time**:

- **37 hours per week

**MAIN DUTIES & RESPONSIBILITIES**

1. To provide a comprehensive Healthy Homes assessment and casework service for clients from initial contact or referral through to the completion.

2. To visit clients in their homes to discuss their needs and ways of meeting those needs including identifying areas of help available to the clients and to provide advice and guidance to clients as required.

3. To supervise the development of each case starting from initial clarification of the options, having due regard for the client’s wishes.

4. To be aware of the progress of current cases so that enquiries can be dealt with promptly.

5. To identify other sources of assistance for clients referring or signposting them to other appropriate support agencies.

6. To use the Care Network and Healthy Home Assessments to carry out assessments for all clients to ensure that all relevant organisations are engaged in joint working with the aim of providing a responsive and integrated service to residents of the borough.

7. To develop and maintain the operation of a system for recording and monitoring which enables the appropriateness and accuracy of advice and information given to be monitored, referrals to be tracked and unmet need quantified and for the work of the service to be monitored and evaluated.

8. To liaise with the Operations and Services Manager, and other staff as well as the client and their families/carers in order to keep them fully informed about the progress of their case.

9. To maintain a case review system by which action required can be brought to the attention of the
Care Network team and ensure efficient progression of each case.

10. To produce and keep accurate reports, performance information and other paperwork as necessary to inform the Operations and Services Manager, Chief Executive and board of trustees and other key stakeholders (including the requirements of funders and commissioners) regarding the development of the project.

11. To maintain the smooth running of the scheme by following office procedures and maintaining accurate manual and computer records on all allocated cases, ensuring the files are up to date and easily understandable by other members of the team.

12. To build and maintain partnerships with identified partners and key stakeholders, promoting good networks and representing the service at meetings and functions as required.

13. To work closely with colleagues and external partners as required to maximise impact to clients and promote prevention and to support key service objectives.

14. To assist in the assessment of risks in connection with the service and to take responsibility for dynamic assessments during home visits.

15. To be caring and understanding to clients’ needs and aspirations, communicating sensitively and effectively with clients both verbally and in writing.
