Despite tackling a global pandemic, which has put our health services under more pressure than we have ever seen in recent times, we have continued to work towards improving outcomes for women and families by forging ahead with the continuity of carer agenda.

We now have three established continuity of carer teams at Lancashire Teaching Hospitals and a plan to develop this further.

An opportunity has arisen for a suitably experienced and highly motivated midwife who is passionate about continuity of carer, service development and quality improvement.

The role is a temporary secondment or fixed term offered for 12 months, it has been offered in this way since 2018 as it is funded by the L&SC Better Births Programme, for the development and implementation of Continuity of Carer.

The Project Lead Midwife will be responsible for leading, developing and coordinating the implementation of Continuity of Carer pathways with clinical excellence, leadership, innovation and effective communication.

Working closely with the maternity workforce and maternity management team, the Project Lead will ensure the delivery of the project aims, objectives and key performance indicators. You will submit monthly data to the regional team and will look at ways to develop the continuity of carer offer across the service.

Leading the service into the future will be a key requirement, working closely with the Consultant Midwife and Maternity Matrons.

**Duties will include, but not be limited to**:

- Leading the realisation of the benefits that come from implementing Continuity of Carer in line with the national trajectories for delivery.
- Marketing and communication of the benefits of new ways of working across the maternity workforce
- To co-ordinate and chair all meetings of the relevant groups involved in the project
- To ensure that all relevant and appropriate staff are involved in the project groups
- To ensure that all relevant data is obtained and available for the groups
- To ensure that all of the developed clinical pathways are in line with principals of CoC
- To facilitate and monitor the completion of all agreed actions
- To ensure effective communication with all stakeholders.
- To monitor progress against the key performance indicators / identified benefits of the project and escalate issues affecting completion as needed
- To ensure effective liaison women in our care
- To produce regular monitoring reports for clinical governance committees as required.
- To plan the evaluation of the project with regard to clinical outcomes as well as patient and staff feedback
- To feedback to and produce reports for the LMS/Workstream as required.
- Attend relevant local and regional meetings or conferences as required
