An exciting opportunity has arisen for an Associate Nurse Band 4 to join our community learning disability team in Northumberland.

The successful applicant will be joining an established and recently expanded Community Leaning Disability Team who work in partnership with Primary and Adult Social Care.

The post base will involve supporting clients within Northumberland.

The post will involve joint working with both Core and Enhanced pathways that support with client needs such as: mental and physical health and behaviours that challenge. The post holder will work with multi-professional practitioners such as Learning Disability Nurses, Psychologists, Psychiatrist and Specialist PBS nurses.

In addition interface with community partners such as the local authority social care staff, third sector care providers and care management.

Applicants must have NMC Nursing Associate registration and experience of working with people with learning disability.

The Nursing Associate is a Nursing and Midwifery (NMC) registered professional with accountability and responsibility for their practice, evidence based decision making, working within the limits of their competence

The Nursing Associate works independently under the leadership of Registered Nurses, working within all aspects of the nursing process.

Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours consistent with CNTW values.

**They will understand and act in accordance with the NMC Code**: Professional Standards of Practice and Behaviours for Nurses, Midwives and Nursing Associates and fulfil all registration requirements.

They will provide compassionate care that is based on empathy, kindness, respect and dignity.

To comply with the NMC Standards of Proficiency for Nursing Associates (October 2018) which relates to the following platforms
1. Being an accountable professional
2. Promoting health and preventing ill health
3. Provide and monitor care
4. Working in teams
5. Improving safety and quality of care
6. Contributing to integrated care Communication and relationship management skills Specific procedures as listed

Provide and receive complex difficult and sensitive information, which may be in a challenging and emotive atmosphere.

Communicate and collaborate with clinical nurse lead, peers, medical staff, patients, families and carers, general public, external agencies, voluntary agencies, GP's, other regional NHS Trusts.

Listen to services users and carers to promote and practise shared decision making

Collaborate with service user/carer groups and representatives to promote service improvement and development.

Prepare and present accurate reports both verbal and written.

Participate in multi-disciplinary reviews.

Communicate with service users and carers in order to positively develop therapeutic relationships

Develop effective interpersonal skills in managing conflict and resolution Adopt a common sense approach to confidentiality

Promote and practice a culture of compassion where relationships are based on empathy, kindness, respect and dignity

Exercise the professional duty of candour, being open and honest and apologizing when things go wrong.

Be registered as a Nursing Associate

Have a Foundation Degree and complete the preceptorship programme for newly registered Nursing Associates

Have experience of working with service users with complex mental health needs or learning disabilities

Demonstrate knowledge of Mental Health legislation

Demonstrate commitment to continuing professional development and Revalidation

Analytical and Judgemental Skills

Contribute to the assessment, planning and evaluation, treatment and risk management for service users

Improve the quality of care by contributing to the continuous monitoring of people’s experience of care.

Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of people first.

Co-ordinating daily routines and activities

Plan tasks and work guided by standard operating procedures and protocols
