The role of the Digital Care Coordinator will be to support patients and healthcare professionals to maximise the potential presented by innovations in digital healthcare technology, and the associated new ways of working. This is achieved by harnessing the power of:

- Wearables and remote monitoring technology
- Data and intelligence - e.g. Population Health data, Dorset Care Record

Digital care coordination is a mechanism for delivering 21stCentury care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.

**Salary**: £22,635.93 per year
