Transfer of Care (Physical) Assistant Practitioner.

Are you looking for the opportunity to make a positive impact on peoples lives through rehabilitation? Supporting the improvement of their physical health, independence and quality of life?

South Bedfordshire are looking for a full time Band 4 Assistant Practitioner to be part of the Transfer of Care Team. 7-day service, 8am-9pm.

Using a personalised and holistic approach, you will support individuals within their own environment, encouraging them to regain confidence and independence through meaningful tasks such as personal care, meal and bedtime preparation and exercise programs. All visits are goal orientated via rehab plans. Developing skills in moving and handling, risk assessing and identifying immediate solutions to reduce risks.

You will be supervised by a qualified member of staff, in order to provide you with support and upskilling.

There are apprenticeship opportunities supported by senior management when opportunities arise.

All applicants will need to have experience of working within a physical therapy service and ideally have community experience. You will also need to hold a full drivers license, have access to a vehicle and be doubly vaccinated for Covid-19.

- Works within the multidisciplinary team in a primary care setting including clients’ homes, residential homes and clinic, providing support for a variety of therapeutic and clinical interventions based on the team skill mix.
- Participate in the assessment, planning, implementation and individualised therapeutic interventions under the guidance of the multidisciplinary team
- To identify and provide appropriate assistive equipment for clients to facilitate safety in their chosen environment
- To work autonomously and undertake lone working in the community
- To work a variety of shift patterns which include weekends and bank holidays on a rota basis.
