**Responsibilities**:
A fantastic, new opportunity has arisen to become an Outreach Worker, engaging with hard to engage sunstance misuers in the Southport & Formby area.

The Outreach Worker role will combine assertive street outreach, supporting partner agencies and the core Substance Misuse Team.

The role will involve supporting individuals to navigate housing pathways and treatment pathways in Southport and Formby to encourage sustained engagement and divert people from rough sleeping and support to access substance misuse treatment.
