Manchester University NHS Foundation Trust are currently seeking experienced Occupational Therapists to provide a high quality, responsive service for patients in the Trafford community, working as part of the Community Rehabilitation Service.

Main duties of the job

As an integral member of service, you will be working within the community helping patients stay in their own home, empowering recovery and preventing hospitalisation.

You will:

* carry out the initial assessment of patients referred to the team, be responsible for providing subsequent specialist assessments to assist in the determination of the medical/clinical diagnosis and formulation of treatment plans, utilising goals agreed with the patient.

* Plan, deliver, evaluate, and modify specialist programmes of care for patients with complex medical conditions /social needs, providing advice to patients and carers in conjunction with the team and other agencies involved.

* Be responsible for the monitoring and evaluation of such programmes to ensure maximum patient benefit, supporting the Senior Practitioner to involve patients in service evaluation and design.

* Support the development of a culture of accountability and governance at all levels

To be successful in applying for this role you will need:

* Current relevant professional registration (HCPC Registration)

* Minimum 2 years post-qualification experience

* Enhanced DBS

* Experience of working within the NHS

* Car driver / owner able to travel to other locations within the trust using own vehicle (mileage paid)

Job description

The post holder will be responsible for the planning and delivery of Occupational Therapy within the designated area. Manage a caseload of service users with complex needs, using evidence based/person centred principles to assess, plan, implement and evaluate interventions.

Occupational Therapy degree or equivalent

HCPC Registered
