Work Arrangement: Hybrid (2 days in the office, 3 days from home)

Job Overview:

Sefton Council is seeking a dedicated and experienced Social Worker to join our Children with Disabilities Team. This role offers a fantastic hybrid working arrangement, allowing you to work 2 days in the office and 3 days from home, providing the perfect balance of flexibility and support. You will play a crucial role in supporting children with disabilities and their families, ensuring they receive the best possible care and services.

Key Responsibilities:

Assess the needs of children with disabilities and their families, developing tailored care plans to meet those needs.
Work collaboratively with multi-disciplinary teams, including health, education, and other social care professionals, to provide comprehensive support.
Conduct home visits and office-based meetings to assess and review care plans, ensuring they remain effective and responsive to changing needs.
Provide emotional and practical support to children and their families, helping them navigate available resources and services.
Maintain accurate and up-to-date records in line with Sefton Council's policies and procedures.
Advocate for the rights and needs of children with disabilities, ensuring their voices are heard and their best interests are prioritized.
Participate in case conferences, reviews, and other relevant meetings to discuss and plan for the welfare of children in your care.
Ensure compliance with relevant legislation, policies, and procedures, including the Children Act 1989 and the Care Act 2014.
Support the team in achieving its objectives and contributing to the overall improvement of services for children with disabilities.Requirements:

Qualified Social Worker with current registration with Social Work England.
Significant experience in working with children with disabilities and their families.
Strong understanding of relevant legislation, policies, and procedures.
Excellent assessment, planning, and organizational skills.
Ability to work effectively both independently and as part of a multi-disciplinary team.
Strong communication and interpersonal skills, with the ability to build positive relationships with children, families, and professionals.
Commitment to promoting equality, diversity, and inclusion in all aspects of service delivery.
Ability to manage a diverse and demanding caseload effectively.To Apply:
