Key Responsibilities

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Clinical
  1. Develop a high level of clinical decision making, monitor and improve standards of care, working within their advanced scope of professional practice.
  2. Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation.
  3. Co-ordinate and manage nurse-led clinics (within Speciality/service).
  4. Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
  5. Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
  6. Support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.
  7. Work with the Matron to promote the development of nurses undertaking the CNO Ten Key Roles as considered appropriate e.g., Nurse Prescribing.
Leadership
  1. Be an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture.
  2. Identify, develop and sustain mechanisms to support patient involvement and feedback.
  3. Be a resource for health care professionals by being visible, available, and accessible for support and advice in relation to the management of patients in the speciality.
  4. Provide leadership to the team of nurse specialists and working with the matron, develop new roles / ways of working (High Impact Changes) linked to directorate business plans.
  5. Lead on consultations and develop strategies for delivering effective care within a changing environment that improves the quality of care and health outcomes in the specialist area.
Management / Operational
  1. Employ effective decision making skills to address complex issues and use effective change management skills to implement these.
  2. Take the lead in the investigation of incidents / complaints when required, including detailed reports / letters to complainants.
  3. Manage a team of specialist nurses / advanced nurse specialists towards achieving directorate and Trust objectives.
  4. Identify and develop proposals and business cases as required for future service innovation.
  5. Manage and develop and influence the development of practice / services in the clinical area through the business planning process / service development and in conjunction with Matron, Managers, Lead Clinician and Assistant Chief Nurse.
  6. Ensure appropriate skill mix, cover is provided when planning rotas and delegating workload.
  7. Delegate work to match capabilities and workload of team members.
  8. Manage own and other resources effectively to budget ensuring quality service delivery within budgetary constraints seeking to reduce budgetary costs by ongoing review.
  9. With team members undertake performance development reviews including reviewing the performance measures framework.
  10. Monitor sickness, absence and competence issues in line with Trust Policy.
  11. Manage recruitment and selection of staff in line with Trust Policy.
Advocate
  1. Ensure that high standards of nursing care are given and maintained and act when standards are not being met.
  2. Involve patients and carers in the planning and delivery of care.
  3. Be the patients advocate and ambassador respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements.
  4. Lead the development of a patient focused education and information giving strategy within the speciality.
  5. Recognise, prevent and diffuse potential areas of conflict and take a key role in the management of the outcome.
Clinical Governance / Quality
  1. Ensure compliance with policies, procedures and clinical guidelines for self and others.
  2. Develop and influence policy making and clinical guidelines internally and externally, necessary to support the specialist nursing service.
  3. Continually monitor standards of care and lead the improvement of care through benchmarking, Nursing Charter Audits and Clinical Audit.
  4. Support and facilitate research ensure evidence-based practice in the specialist area.
  5. Disseminate good practice and knowledge by pursuing opportunities to present work at national conferences or via publication, encourage team members likewise.
  6. Promote patient and public involvement activities in the specialist area leading to service improvement.
  7. Promote people's equality, diversity and rights.
  8. Identify clinical issues and incidents within the Trust that reduce the quality of care within the specialist service bringing any issues to the attention of the Matron.
  9. Take part in activities to counter these to ensure the delivery of safe and effective care.
  10. Participate and take the lead in clinical audit within the specialist area supporting others to participate and ensuring agreed results are implemented.
Education / Training
  1. Take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
  2. Participate in the supervision and the teaching of pre-post registration students, new staff members and junior medical staff.
  3. To lead and develop with the Matron educational programmes in relation to the speciality.
  4. Support the ward / department sister in ensuring the clinical environment is conducive to effective learning.
  5. Identify training needs and develop training resources for nursing staff to enable role development and to meet the needs of the service.
Professional Responsibility
  1. Maintain active status on NMC register.
  2. Act always in accordance with NMC code of conduct and guiding documents, Health Care Standards.
  3. Adhere to Trust Policy & Procedure.
  4. Maintain a professional portfolio.
  5. Maintain a high level of awareness of relevant research issues and trends within the specialist area of practice and in nursing generally.
  6. Take an active part in the development of nursing in the Trust via participation in professional nursing forum and shared governance initiatives.
  7. Take responsibility for personal development and education with regard to, Personal Development Plan objectives.
  8. Participate in Clinical Supervision for self and others and utilise reflective practice techniques.
  9. Provide support and development for nurses through mentorship.
  10. The post holder is responsible for ensuring the safety of patients at all times and promoting a safety culture through the effective management of risk.
Area of Work

The post holder will be required to undertake training events at any site across the trust. To support maintaining clinical skills, the post holder will be required to work clinically on base wards in the division. This is likely to be equivalent to a minimum of one session per week i.e., 4hrs in the CNS job plan. In periods of high activity when base wards need additional support the time commitment may increase. At that time, this will be risk assessed by the relevant Matron who will consider the impact on the individual's workload and the relevant service.

Staff Development, Training and Education

The post holder will be required to undertake mandatory training and is responsible for keeping this training up to date. The post holder will have an appraisal of performance each year and will be responsible for agreeing a development plan in line with the Trust's Framework, in agreement with their manager or immediate supervisor. The development plan will be reviewed each year. The Trust will provide assistance and agreed development to enable the post holder to achieve their objectives and standards in line with the development plan. If the post holder feels he/she is not achieving their objectives as agreed in the development plan they will bring it to the attention of their supervisor or manager at the earliest opportunity.

Health and Safety

Work in accordance with Health and Safety regulations at all times. Report any incidents of breaches of Health and Safety and report any dangerous acts or omissions that are seen in the course of duty that compromise the Health and Safety of staff or patients using the Trust Health and Safety policy. Comply with audit recommendations and risk assessment recommendations to make the workplace and work practice safer. Assist when required to do so, in any risk assessment activity undertaken.
