Liaison Nurse for Complex Needs

A fantastic opportunity has arisen for a 12 month secondment/fixed term contract, to work in the Complex Needs Team. The Complex Needs Team is a fast paced, forward thinking, and a national award-winning team. If you want to expand your knowledge and have the opportunity to be part of an innovative service, then this is the role for you.
The complex needs service covers learning disabilities, autism, dementia, delirium, and mental health. The post holder will support the whole complex needs agenda.
Main duties of the jobUnder the supervision of the Lead Nurse for Complex Needs you will promote a quality service using research and evidence-based practice including the setting, monitoring, facilitating, and auditing of clinical standards/pathways.
You will support improvements in experience and quality of care, for patients with Complex Needs, by supporting a holistic approach to care and ensuring the physical needs of patients are addressed. This is achieved by supporting patients in a planned and unplanned capacity to receive excellent patient experience each and every time.
Job responsibilities- Support staff with complex cases; while working with clinical staff, the Liaison Nurse will demonstrate through patient care how to improve the care and treatment of patients with a complex need.
- The post holder will work under the supervision of the Lead Nurse for Complex Needs and co-ordinate their activities to ensure that an efficient service is provided.
- Support the development of practice with service leads and others as appropriate.
- Work closely with all members of the Complex Needs Team to ensure the teams objectives are met.
- Maintain excellent communication networks and relationships within and externally to the Trust.
- Work with minimum supervision and coordinate own activities to ensure that an efficient service is provided.
- Uphold the Trusts values and behaviours (available on the Trust website) and behave in a manner fitting with the responsible position of the post holder, maintaining public confidence.
- Maintain professional and courteous working relationships with staff and respect the equality and diversity of each and every person he/she comes into contact within the course of his/her business.
- Uphold the privacy and dignity of the patient and respect the equality of patients at all times.
- Ensures that own practice demonstrates current clinical knowledge, credibility, and best practice. Acts as a role model and be recognised as an expert by colleagues.
- Registered Nurse.
- NMC Registration.
- Recognised teaching qualification post-registration qualification or experience of teaching.
- Extensive clinical experience caring for patients with one of the complex needs (Learning Disability, Autism, Dementia, Delirium or Mental Health).
- Evidence of continued professional development.
- Experience of audit, research, and evidence-based care.
- In depth specialist knowledge into at least one of the Complex Needs.
- Awareness of safeguarding agenda and mental capacity act agenda.
- Teaching / assessment skills Organisation and negotiation skills.
- Excellent verbal, written, interpersonal and communication skills.
- Evidence of audit and change management ability.
- Ability to motivate self and others.
- Evidence of leading and facilitating change.
- ECDL.
