Specialist Dietitian - Diabetes/ESD

Grade: AFC Band 6 (or Band 5 to 6 career progression opportunity, depending on experience)

This is an exciting opportunity for an experienced Dietitian. This post will enable you to build on and increase your skills and experience within the speciality of diabetes and also to work within the stroke early supported discharge multi-disciplinary team in the Hambleton and Richmond locality.

Main duties of the job

The post holder will work within the established diabetes services based mainly at the Friarage hospital and will also work within Hambleton and Richmond community area both within the ESD team and diabetes team. The postholder will be involved in developing the service alongside other dietetic colleagues and health professionals. Caseloads will include people with type 1 and type 2 diabetes and those with gestational diabetes. You will work alongside, and be supported by, senior specialists in diabetes to deliver dietary advice in a range of clinics as part of an MDT including young adolescent clinics, insulin pump clinics and dietitian led clinic. Within community setting venues you will deliver the structured education programme for those with type 2 diabetes and with time this may expand to support with the delivery of structured diabetes for type 1 diabetes. You will also have the opportunity to work with the diabetes team based at The James Cook University Hospital one day per week.

As part of the early supported discharge team you will support patients in a smooth transfer between the inpatient stroke services to the home setting, providing dietetic input as part of the wider multidisciplinary team.

There may also be times where you will be required to provide dietetics services at the James Cook Hospital or South Tees Community Dietetics department as part of the wider South Tees NHS Foundation Trust Nutrition and Dietetic Service for diabetes.

Knowledge and Skills (Band 5 and 6)

Essential


  • Evidence of audit and participation in service development
  • Skills in the use of SystmOne or another electronic patient record system

Desirable


  • Membership of: British Dietetic Association (BDA), BDA specialist groups and other relevant groups

Qualifications and Training (Band 5 and 6)

Essential


  • To hold a Degree in Dietetics or equivalent
  • To be registered as a Dietitian and fit to practice by the Health and Care Professions Council (HCPC)
  • To have achieved or willing to work towards relevant post-graduate qualification

Qualifications and training (Band 6 criteria)

Essential


  • Clinical supervisory skills training
  • Able to evidence completed post-graduate preceptorship programme or equivalent competencies
  • Leadership training

Qualification and Training (Band 5 criteria)

Essential


  • Able to evidence progress with post-graduate preceptorship programmes or equivalent competencies

Desirable


  • Clinical supervisory skills training

Experience (Band 5 and 6)

  • Recent NHS experience in a clinical setting
  • Recent experience of managing patients requiring tube feeding.

Desirable


  • Experience of supporting patients following a stroke.
  • Experience of supporting patients with discharge process between hospital and home

  • Can demonstrate relevant/varied experience working across a variety of clinical areas including Type 1 and Type 2 diabetes.

Desirable


  • DESMOND training or alternative
  • DAFNE trained or alternative
  • Experience of working within specialist diabetes areas, such as antenatal care, insulin pumps

Experience (Band 5 criteria)

Essential


  • Can demonstrate post graduate experience working within a variety of clinical areas including some nutritional support and diabetes

Desirable


  • Can demonstrate relevant/varied experience working within more specialist clinical areas including Type 1 and Type 2 diabetes.
  • Experience of delivering group education to patient groups
