This is an exciting and new opportunity to work as part of the Integrated Discharge Team, supporting the inpatient teams to implement the Hospital Discharge policy with the 'Home First' ethos and as a result we are looking to recruit full time Band 6 Occupational Therapists or Physiotherapists to work as part of the team.

The philosophy of the Integrated Discharge Team is to provide holistic, highly skilled, integrated assessment by an experienced multi-disciplinary team to ensure patients are discharged from acute care in a timely manner, reducing delays and supporting the care closer to home agenda.

This exciting role will work as part of the Integrated Discharge Team, working closely with both the hospital and community teams to support and facilitate timely, functional assessments and discharges for patients to reduce length of stay and harms. Furthermore, facilitating safe, effective discharge to the most appropriate environment. Although based within the Integrated Discharge Team, you will be professionally supported by the wider AHP team based within Bury Care Organisation.

There will be a requirement to work weekends/bank holidays.

Main duties of the job


  • Work within the Integrated Discharge Team at Fairfield General Hospital
  • Work in line with the agreed strategic objectives of Bury Care Organisation
  • Utilise high levels of clinical assessment and communication skills to support effective discharge planning, reducing unnecessary hospital stays and to improve flow through the acute hospital pathway.
  • Work autonomously within trust policies and professional practice and maintain accurate and comprehensive patient records.
  • Perform specialist assessment of patients and determine a clinical diagnosis, develop, implement, evaluate and modify individualised treatment programmes, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. The patient will present with acute or chronic conditions, some of which may be complex.

Person Specification

Education / Qualifications

Essential


  • Degree in Occupational Therapy/Physiotherapy or equivalent.
  • HCPC registration
  • Evidence of CPD in the form of a detailed personal development portfolio.
  • Evidence of relevant post graduate training

Desirable


  • Member of the British Association of Occupational Therapist (BAOT) or The Chartered Society of Physiotherapy (CSP)
  • Member of special interest group(s)
  • Fieldwork Educator accreditation

Experience

Essential


  • Experience of working as an Occupational Therapist or Physiotherapist
  • Experience of working within A& E/Frailty/Intermediate Care/Crisis Response
  • Experience of integrated / inter-professional working.

Desirable


  • Experience of working in the community
  • Leadership
  • Audit

Knowledge

Essential


  • Knowledge of a range of therapeutic procedures underpinned by evidence based practice
  • Specialist knowledge in anatomy and physiology to underpin relevant clinical skills
  • Have specialised clinical knowledge and experience relating to a wide range of clinical conditions

Desirable


  • Knowledge of research and development.
  • Knowledge of relevant legislation and the impact on practice

  • Well-developed written and verbal communication skills with people from a wide variety of backgrounds
  • Able to convey complex information in an understandable way clearly and accurately
  • Key board skills (word - processing, internet, data inputting )
  • Proven organisational skills including time management, effective delegation and prioritisation and the ability to complete a task
  • Ability to work within a clinical area with senior support but without direct supervision
  • Ability to provide appropriate supervision, support and delegation of work to relevant staff.
  • Ability to plan and prioritise activities and workload for self and others
  • Ability to carry out assessments in a range of clinical settings
  • Establish positive working relationships with patients' relatives, colleagues and other agencies.
  • Ability to undertake often complex risk assessments and plan care accordingly.
