This is an exciting and new opportunity to work as part of the Integrated Discharge Team, supporting the inpatient teams to implement the Hospital Discharge policy with the 'Home First' ethos and as a result we are looking to recruit full time Band 6 Occupational Therapists or Physiotherapists to work as part of the team.
The philosophy of the Integrated Discharge Team is to provide holistic, highly skilled, integrated assessment by an experienced multi-disciplinary team to ensure patients are discharged from acute care in a timely manner, reducing delays and supporting the care closer to home agenda.
This exciting role will work as part of the Integrated Discharge Team, working closely with both the hospital and community teams to support and facilitate timely, functional assessments and discharges for patients to reduce length of stay and harms. Furthermore, facilitating safe, effective discharge to the most appropriate environment.
