Myeloid Senior Specialist Nurse

An exciting opportunity has arisen for an experience nurse to join the Myeloid multidisciplinary team based at the Churchill Hospital in Oxford. The Myeloid MDT provides care for local patients and tertiary level care for the Thames Valley region. The team includes a Band 6 specialist nurse and clinical and academic medical staff . The post holder will liaise with the inpatient, outpatient, ambulatory, BMT and research teams, primary care and regional hospitals. Patient-centred care is our priority, so our patients receive high class, personalised treatment.
Patients looked after by the team include those with acute leukaemia (AML and ALL) together with a range of myeloproliferative disorders including CML, myelofibrosis, essential thrombocythaemia and polycythaemia. The treatment of these disorders is going through an era of rapid progress and development this gives the post-holder the opportunity to support and develop a world-leading clinical service for this group of patients.

- Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and discuss potential diagnoses with members of the wider MDT.
- Decide on and carry out treatment, which may include advice on the prescribing of medicines, or referring patients to an appropriate specialist.
- Assess individuals, families and populations holistically using a range of different methods, using physical assessment and advanced history taking skills and other specialist health needs assessments.
- Ensure the provision of continuity of care.
- Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed.
- Work autonomously within a wider health care team and as a lone practitioner.
- Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.

Clinical practice
- Work at an advanced level of practice within the specialist area of practice, autonomously as well as within a multi-disciplinary team and are self-directed.
- Work clinically to optimise health and wellbeing, demonstrate autonomous decision making related to comprehensive history taking and physical assessment of patients.
- Work at an advanced level drawing on a diverse range of specialist knowledge in complex decision making to determine evidence-based therapeutic interventions, which may include advising the wider MDT on the prescribing of medication.
- Plan, prioritise and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, providing direct support to patients and clients.
Leadership and management:
- Collaborate with the multi-professional team to develop a shared vision of the service.
- Act as a role model and resource in expert clinical evidence-based practice.
- Work collaboratively with colleagues to develop effective documentation, pathways, protocols and guidelines for care.
- Demonstrate a commitment to share expertise and disseminate information.
- Actively participate in the development of the team and the service to continuously improve patient care
Education and training:
- Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through ongoing self-education and attendance at relevant courses of study.
- Develop and maintain a peer network of support, information and learning with other nurse specialists at a local and National level.
Improving quality and developing practice:
- Coordinate and participate in the service quality meetings which promote ongoing service review and evaluation, including seeking and monitoring patient experience and outcomes
- Participate in research / audit to generate new evidence in area of speciality.
- Evaluate clinical practice in relation to its evidence base and clinical effectiveness including policy and guidance
- Identify areas of risk and poor quality, and address these through appropriate governance structures and forums.
- Registered nurse (part 1, 8, 12 or 15 of register depending on area of practice)
- First level degree (HEI level 5 or 6) in nursing, midwifery or equivalent
- Completed or completing Master's level (HEI level 7) qualification
- Relevant specialist post basic programme
- Teaching / Assessment / Mentoring qualification
- Experience in an advanced nursing role
- Evidence of continuing professional development
- Previous experience of formal/informal teaching of patients and staff.
- Demonstrable expertise in haematology nursing
- Previous experience of formal/informal teaching of patients and staff.
- Management skills / training
- Experience in developing and managing nurse-led clinics and practice
