Children's Community Specialist Nurse

Bradford, United Kingdom Bradford Teaching Hospitals NHS Foundation Trust Full time

A rare and exciting opportunity has arisen for a highly motivated and experienced paediatric nurse to join our multi-disciplinary team. We are a well-established service caring for both local children and those in the wider Yorkshire region.

We are linked to the new born Screening Service in Leeds which covers all babies born in Yorkshire. Our base is St Luke's Hospital in Bradford but a lot of our work is undertaken in the community at the family home, nursery or school. We have three MDT out-patient clinics each week. We endeavour to support the child and family in their home and implement specialist treatments when necessary to prevent hospitalisation. As part of our team you will educate and support families at diagnosis and throughout the child and families' journey including transition to Adult Services. You will support the child, family and local teams in the child's community. We will support you to build upon your existing knowledge, working as part of our IMD team to deliver a service that is led by the needs of the child and family.
You will have responsibility for a defined caseload (patients/service-users) and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers.

You will provide clinical, social and psychological support to their patients/service-users using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health.

Our People Charter outlines the behaviours we can expect from one another and what you can expect from Bradford Teaching Hospitals Foundation Trust:
