The Targeted Lung Health Check (TLHC) programme will enable the early detection of lung cancer in order to save lives. The post holder will be expected to provide specialist knowledge and care to the patient and their carer's. They will support colleagues in the effective management of patients in their specialist area who require a high level of judgement discretion and decision making within an outpatient setting. The post holder will provide education and training to patients, carers and health care professionals.

We provide specialist services in cardiothoracic surgery, cardiology, respiratory medicine both in the hospital and out in the community.

Manage referral of patients who are identified as having a clinical requirement and identifying when a patient requires medical attention.


Involve patients, carers and relatives in the giving of information regarding the project.


Undertake training to be able to complete patient assessment, shared decision tool and be able to facilitate appropriate on-going referrals.


Provide sensitive, complex information to patients, carers, relatives and professionals regarding the project.


Ensure onward referral to other relevant care providers ie smoking cessation, pulmonary rehabilitation, etc


Observe the provisions of and adhere to all Trust policies and procedures.


Actively participate in the annual performance review to identify personal development needs.


The employee must attend Infection Control Induction training and mandatory Infection control training. The employee must follow all Trust policies, procedures and guidelines relating to Infection Control.


Adhere to the relevant Code of Practice of NMC.


Exercise judgement, discretion and decision making in clinical care in line with the project protocol.
