Job summary

An exciting new post has arisen within our Burmantofts, Harehills and Richmond Hill Primary Care Network (BHR PCN).

We have a holistic approach in managing our patients who are culturally diverse and many living with deprivation. We are friendly innovative network of 12 GP Practices serving a patient population of 70,000 including patients living across 6 care homes. The post holder will be expected to work across all practices in the PCN. We will support the candidate to complete the mandatory Dietetic HEE roadmap training to become a First Contact Practitioner. You will be part of an organisation whose leadership team are supportive and innovative and focused on change and transforming services. Diabetes and Cardio-vascular disease is a particular focus for our PCN.

We are looking to employ a dietitian, working alongside the PCN Clinical team and the practice clinical staff. The post holder will be enthusiastic and want to make a difference. They will support patients in achieving changes to improve their personal health and wellbeing. 

Main duties of the job

Diabetes and Cardio-vascular disease is a particular focus for our PCN.

To address the healthcare needs of our local population we are hoping to employ a dietitian across our PCN. Working alongside the PCN Clinical team and the practice clinical staff, the post holder will support patients in achieving lifestyle changes to improve their personal health and wellbeing. 

The post holder will be expected to work across all practices in the PCN, undertaking clinics and potentially working in innovative ways.

Supervision and mentoring will be provided by appropriate medical staff who have completed the train a trainer course - the type of support will depend on the post holders skills and knowledge and determined by our clinical governance arrangements.
