Physiotherapist Community Rehab and Falls

Integrated care is all about providing better care and support to our patients, users and the people of Salford by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.
You will join our multidisciplinary Community Rehab and falls team carrying out holistic assessments of patient needs and develop, implement and evaluate treatment programmes. There is a varied caseload to support adults and older people within their own homes, including carehomes.
You will be an autonomous practitioner and work as a key member of the multidisciplinary team and supervising junior staff.
You will have an active role in coordinating new referrals, managing waiting lists and supporting the team to maintain patient flow.
You will have a broad range of experience managing medical, surgical, neurological and mental health conditions and ideally some community experience.
The service is city wide and would require you to be able to travel to various locations across the city of salford on a daily basis. - HCPC registered
- fieldwork educator
- Significant post graduate NHS experience including experience of working in all core clinical areas

- Experience of working in the community
