MAIN PURPOSE:
The role provides crucial capacity tosupport the system, with a focus on LBB services, provision andpathways to respond to the increased winter demands. The WinterProgramme Lead (LBB) will manage the LBB winter funding ensuring it isdirected to meet presenting demand, monitoring and tracking impact.Provide crucial challenge to the system and leading transformationalwork to ensure clients are at the centre of all we do and resource ismaximised.

SUMMARY OF RESPONSIBILITIES AND DUTIES:

  • Manage LBB winter funding, deploying it to meet presenting needsensuring increased capacity meets need, is delivered within thefinancial envelope and achieves maximum impact
  • Developwinter capacity and demand live dashboard to allow system monitoringand management
  • Identify system challenges, leadtransformation and change
  • Developing social care pathwaysto reduce pressures on acute based care
  • Programme managethe roll out if re-directed acute funding, harnessing innovation andenhancing the current national best practice to gold standard model ofcare
  • Work with senior leadership appropriately escalatingand providing solutions and leading change
  • Developingintegrated models of care that can support the system to respond tosystem pressures
  • Develop and mainstream a long term modelthat maintains current performance mobilising Trusted Assessor modelto maintain performance
  • Provide regular reporting on impactand outcomes

KNOWLEDGE

  • Knowledge of health and social careand the discharge system
  • Knowledge of national policyrelating to hospital discharge
  • EXPERIENCE
  • Experience of working in complexmultiagency systems working around discharge and admission avoidance
  • Experience of staff management and supervision
  • Experience and/or ability to work at all levels within theorganisation
  • Communicating effectively with a range ofstakeholders
    Experience of achieving change in complex system

QUALIFICATIONS

  • Be educated to a minimum of firstdegree level or relevant equivalent work experience
