To lead the therapists within a frailty service within HMR.

To perform specialist assessment and treatment of patients within the frailty service/home in a day service

To promote a home first approach to SDEC and the frailty service.

The post holder will be expected to lead and co-ordinate the therapy to the frailty service and be professionally accountable for the delivery of patient care. They will act as a clinical lead / specialist for patients requiring physiotherapy/occupational therapy within the frailty/sdec units.

Main duties of the job

The therapists will promote a home first approach to SDEC and the frailty service.

The role will ensure and support the delivery of expert clinical care for the physiotherapists/occupational therapists on the team. Leadership will be required to facilitate professional specific supervision and training and also to educate the teams.

The post holder will have continuous responsibility for the management of risk, demonstrating flexibility and adaptability to work safely in different environments. They will be required to work autonomously and manage day-to-day activities of the service but also provide consultation as required with other health and social care professionals involved in the care of HMR adult patients and their carers.

The post holder will carry a case load. Take a lead role in focussing team resources to meet the identified health needs within the services.
