This is an exciting opportunity to join the Home Treatment team (SHTT) in Stockport as we are currently undergoing a transformation in service delivery and care. We are looking to recruit an Occupational Therapist with experience in Mental Health to work in the HTT.The HTT is a multi disciplinary team with staff from a range of professions including, mental health nurses, occupational therapists, social workers and support workers. The core purpose of this role is to work within the Home Treatment team to embed the Occupational Therapy assessment and intervention pathway across the service within the framework of Model of Human Occupation (MOHO), in collaboration with the wider MDT staff and work towards a recovery focused ethos.


The post holder will be highly skilled and competent in conducting OT assessments developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures.

Undertake assessment to determine the need for longer term support (e.g. referral on to CMHT) under the Care Programme Approach.
Participate in risk assessment and formulation of risk management plans
Provide supervision to staff in the team according to trust policy and standards.

Being sensitive to their needs for respect, privacy and dignity and to act as a senior point of contact on the ward.
To ensure that high standards of record keeping, patient assessment, care planning, risk assessment and management are delivered.
To delegate appropriate duties to junior registered and support staff, providing relevant direction and oversight.
To assist in the investigation of complaints and clinical incidents within the community mental health team.
To demonstrate a working knowledge and application of Trust Policies, Guidelines and Protocols, actively contributing to their development and ensuring they are effectively delivering a high standard of clinical care.

Education, Training and Development


To ensure that you fulfil all statutory and mandatory training requirements meeting the requirements of registration with HCPC.
To participate in clinical / professional and managerial supervision and provide practice / clinical supervision to designated registered and non-registered staff within the community mental health service.
To support the acute services manager in the development, organisation and delivery of training specific to the service.

Diploma / Degree in Occupational Therapy
Short courses and experience to post graduate diploma level
Willingness to undertake further training relevant to management, leadership and practitioner roles resulting from legislative or service changes, as appropriate

Evidence of post qualification academic achievement in the relevant area for this post
Training in MOHO

Substantial post registration experience working in a mental health service
Experience in working with people with severe and complex mental health needs
Experience of working in a community setting
Extensive knowledge / experience in social inclusion and recovery model of care
Sound understanding of current local and national policies in relation to Mental Health Act, Risk Assessment and Management processes and CPA processes
Good working knowledge of assessment, care planning, implementation and evaluation processes
Comprehensive knowledge of mental health conditions and how they impact on an individuals wellbeing and occupational functioning
Comprehensive knowledge of broad range of interventions required to support individuals with severe and complex mental health needs

Knowledge of the occupational context of community mental health services and its application to assessment and treatment

Ability to supervise junior staff / students
Ability to value diversity and respond constructively to discriminatory behaviour
Ability to provide professional expertise in the care of mental health service users
Ability to provide appropriate education and training to peers and assistant as approprate
Computer literacy

Willingness to work across the community mental health service
Willingness to attend meetings / training within the Pennine Care footprint and beyond
Use of a car to access to a means of mobility to travel across the Trust footprint in line with service needs
Willing to carry out all duties and responsibilities of the post in accordance with the Trust's Equal Opportunities and Equality and Diversity policies
