The Role
Job Title: Hospital Inreach Link Worker
Hours: 37.5 hours per week
Shift Pattern: Monday to Friday, 9am to 5pm
We are looking for a Hospital Inreach Link Worker to join our Safe Spaces team in Gloucestershire.
The P3 Safe Spaces service provides a short term multi-agency assessment in a safe environment. Patients admitted to emergency departments at Gloucestershire hospitals have a much lower rate of re-admission when they engage with P3 accommodation and assessment services.
Youwill be the single point of contact for homeless patients admitted to Gloucestershire hospitals, ensuring that effective supportis provided to anyone admitted to emergency departmentswith no fixed place stay. Youwill work closely with other P3 services and wider partner agencies to facilitate access to suitable accommodation and support.
We are looking for someone who is flexible in their approach and confident in managing their own workload in an environmentwhere no two days are the same. You will be able to effectively communicate with a variety of agencies including Hospitals, Local Authorities, P3 Services, amongst many others. Knowledge of support services in the county would be an advantage but you will be provided with full training.
