An exciting opportunity has arisen to join the Patient Safety Team as Quality Improvement Investigator.

Reporting to the Quality Improvement & Investigation Lead, as part of the wider Patient Safety Team, this post is responsible for the delivery of the Trust's Quality Strategy and vision to ensure 'Excellent Care for all' through the delivery of safe, effective and high quality services, that ensure the people who use our services, their families, carers and visitors receive a positive experience.

Main duties of the job

Under the direction of the Quality Improvement & Investigation Lead the post holder will be responsible for providing leadership and expertise to support the review and investigation of those affected by patient safety events.

This will include the completion of Duty of Candour as required and actively engage with patients, families, carers and staff to seek input into the learning response and develop a shared understanding of what happened.

The Quality Improvement Investigator will be expected to liaise with those who have been affected by patient safety events and to produce meaningful learning responses for continuous quality improvement in line with the Patient Safety Incident Response Framework (PSIRF).

There will be a test/presentation at interview.

Person Specification

Education

Essential


  • Degree/management qualification or equivalent experience. GCSE (or equivalent) English Language and Maths to grade C
  • Incident Investigation Training or equivalent
  • Evidence of continuing professional development

Desirable


  • Quality Improvement Qualification
  • Previous Experience within the NHS / clinical registration
  • PSIRF/Patient Safety related training

Knowledge and Understanding

Essential


  • Understanding of Patient Safety Incident Review Framework (PSIRF)
  • Understanding of Systems Engineering Initiative for Patient Safety (SEIPS)
  • Understanding of Human Factors in patient safety
  • Knowledge of Duty of Candour regulations

Desirable


  • Experience and working knowledge of CERNER
  • Experience and working knowledge of an incident reporting management system e.g. Radar
  • Application of PSIRF, Learning Responses and Safety Actions

Skills and Abilities

Essential


  • Excellent verbal and written communication skills
  • Investigative skills and ability to remain objective
  • Delivering training/presentations
  • Ability to write reports with a high level of written English language

Desirable


  • Delivering results through others
