An opportunity has arisen experienced, enthusiastic and skilled Community Mental Health Practitioners to join our busy Community Mental Health Team for Older Adults. Full time and part time hours available.


We are looking for someone who: enjoys and works well in a team setting and is also able to work independently is committed to working with older people with mental health needs and is able to work effectively in this demanding role.
As a Lead Professional, you will provide evidence based interventions, risk assessment and management to a broad range of people with functional and organic mental health needs. You will be responsible for the assessment, planning, implementation and evaluation of care needs and be responsible for the clinical management of a defined caseload. This integrated team provides a Memory Assessment Service, care for patients with more complex needs associated with both functional and organic illness, and Care Home and Hospital Liaison. The unique set-up of these integrated teams allows a seamless delivery of care for patients throughout all stages of their illness, and a collaborative approach within the team.


There is a visible and supportive leadership team who are committed to clinical supervision and professional development.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.


The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.

RMN / OT / Social Work qualification
Current professional registration with the NMC / HCPC
Willingness to undertake further training and evidence of continuing professional development

Comprehensive knowledge of mental health needs for older people
Comprehensive knowledge of the assessment and treatment of dementia
Post qualifying experience of working with older adults with mental health needs
Experience of managing a caseload and care coordination
