Location: Shipley Pay Rate: £26 per hour (assignment rate) Contract Type: Locum, 6-month contract/ongoing block booking. Working hours: Monday – Friday, 37.5 hours per week (hours flexible between 8am and 8pm). Part time will be considered. Main Duties: Manage a caseload of clients in the community Input data in the clinical information system: e.g. including risk assessments, care plans, CPA, crisis and contingency plans Administer intra-muscular medication and monitoring treatment plans Work as a part of duty service on the single point of access triaging referrals Supervise and mentor student nurses Demonstrate effective networking skills and ensure a holistic approach to meet service user needs. Work to ensure the safety and wellbeing of service users and their families Work in partnership with service users and carers to achieve goals and to support recovery and reintegration within communities Provide appropriate feedback and liaison with MDT on all aspects of service user care. Ensure that agreed standards of nursing care within the team are achieved. Actively participate in the flexible formats of team supervision and support. Initiate, develop and assist in the trial of new ideas for the improvement of services to service users in conjunction with the multidisciplinary team. Ensure that nursing policies are implemented and maintained Comply with legal and professional requirements Have ability to suggest and critically explore new concepts Have ability to work in a supported autonomous manner within the team Have ability to plan and prioritise workload Have ability to identify, facilitate and support developments and initiatives within the service delivery of nursing and multi-disciplinary team Work within a fully integrated and jointly managed Community Mental Health Team alongside Local Authority social care staff and managers TAssess and provide social care services to service users within the Care Programme Approach and in line with the integrated operational policies.
