Risk Screening Clinician
Remote
Compensation:
£Safeguarding Lead
ProblemShared is seeking an experienced clinician to join our growing Clinical Governance and Risk Team. We are looking for a motivated individual who wants to work in a high-performance team at a scaling health technology company. The post holder will be part of a team who are responsible for ensuring all clients that present with clinical risk are reviewed and supported throughout their journey with us. Our digital journeys are right at the heart of what we do, and our goal is to offer seamless digital experiences to support exceptional healthcare.
As a Risk Screening Clinician, you will be part of a clinical team who oversee all clients as they join our services and require screening for clinical risk. You will be responsible for monitoring our waiting lists and supporting clients at risk, through telephone call screening and supported onward referral.
You will support the Safeguarding Lead and Clinical Team to identify clients in need of further screening or risk assessment, at points in their journey, as well as those who may be unsuitable for an online assessment or intervention. You will carry out screening and assessment at enrolment and every 12 weeks, for clients who have indicated risk and are on our waiting lists. You will keep up-to-date records in our digital systems so that any clinician interacting with the client is immediately informed of their risk status and can easily access concise, timely records of the nature of the risk, management plans and actions taken.
Ultimately, you will support the Safeguarding Lead with companywide risk management training and process development.
This role requires empathy, skills in people and relationship building, organizational skills, confidence and clarity of thought in clinical decision making and understanding of when to escalate concerns both internally and externally. The role is crucial to the development and growth of our clinical services and ensuring quality risk and safeguarding for children and adults at risk within the service.
ProblemShared takes a person-centered approach to neurodevelopmental assessments, post diagnostic interventions and talking therapy services and is building a team of experts who believe in providing care which focus on both strengths and challenges.
ProblemShared services a number of NHS contracts, universities and insurers to deliver high quality ADHD and autism assessments as well as assessments for learning differences. We also have a growing talking therapy service that provides counselling and psychological therapy for students, psychologically complex clients and those with a neurodevelopmental condition.
Excellent knowledge of clinical risk assessment, management and safeguarding
~ Minimum Safeguarding Level 3 and prepared to undertake training to Level
~
~ On a daily basis, you will be responsible for monitoring new clients signing up with ProblemShared and be the first point of contact for those who have been flagged as Amber risk or have indicated they may be unsuitable for an online assessment or intervention.
Using the RAG rating approach, you will engage with the client by telephone and discuss the risk indicated, to assess whether a full risk assessment is required. In such instances, you will carry out the risk assessment by telephone and document the outcome in the Patient Record.
You will be part of a team who ensure that all clients indicating Amber risk are contacted within 5 working days.
You will use your clinical judgment and supporting risk assessment documentation to assess whether the client is Green risk and can move on to our waitlist, Amber risk and requires a management plan and actions to be taken before discharge, or Red risk, requiring immediate escalation to local emergency services, mental health or crisis services and is unsuitable for an online service.
You will work with the Safeguarding Lead to support clients who are at risk and ensure onward referral.
You will ensure you employ a consistent approach to risk screening and complete relevant documentation in a thorough and consistent manner.
Work within the framework of your regulatory body, and ensure professional registration is maintained.
