'Together, WE Can' is an innovative, integrated service model supporting young people who are in residential care or on the edge of care. Our aim is to support young people to live within their own family networks wherever possible, by providing a multi-agency approach and focusing on prevention, reunification and supporting young people, in a planned way to move on into independence.
Based within the residential children's home, you will work within a multi-disciplinary team and invest and promote a relational approach to supporting young people. You will work on an individual basis with young people as well as in groups in delivering a psychosocial model of support.
You will integrate yourself within the support team, sharing the living space and daily routines of young people, providing a natural, relational approach in the delivery of your interventions. Provides highly specialist psychological assessment of young people with highly complex conditions based on the appropriate use, interpretation and integration of data from a variety of sources.
Responsible for identifying and reporting concerns regarding the safeguarding of children who may be at risk.
Undertakes risk assessment and risk management for young people and provides specialist advice to other professionals on psychological aspects of risk assessment and risk management.
Formulates plans for highly specialist psychological therapy and/or management of clients' mental health conditions based upon an appropriate conceptual framework and evidence-based practice. Negotiates the implementation of such plans with young people and the residential team.
Implements a range of highly specialist psychological therapies for individuals, carers, families and groups, adjusting and refining psychological formulations as required.
Undertakes highly skilled evaluations and makes decisions about care and support options. Provides highly specialist psychological advice, guidance and consultation to other staff contributing directly to young people's diagnosis, formulation and intervention plan.
This is a joint initiative between Tees, Esk and Wear Valleys NHS Foundation Trust and City of York Council - bringing health and social care together to meet the needs of young people.

The post holder will have a proven track record of working with vulnerable and care experienced children, young people, families and carers, in health or local authority settings, many of whom have experienced trauma and can have multiple, complex needs. We are looking for someone creative and flexible to work in partnership with health and social care.

Post-graduate training (doctoral level equivalent) in clinical / counselling / forensic psychology (or its equivalent prior to 1996) as accredited by the BPS.
Post doctoral training in one or more additional specialised areas of psychological practice
Trained in clinical supervision and the supervision of doctoral trainees
Trained in care co-ordination
Evidence of continuing professional development
Working as a qualified applied psychologist within the designated service including a wide variety of client groups, across the whole life course and a range of clinical severity across a range of care settings.
Exercising full clinical responsibility for clients' psychological care as a care coordinator within the context of a multidisciplinary team.
Teaching, training and clinical supervision and field supervisor for doctoral theses.
Quality and service improvement and evaluation.
Provide effective teaching, training and clinical supervision for the multidisciplinary team.
Work effectively as part of a multidisciplinary team and undertake care co-ordination/lead practitioner responsibilities
