We are looking for an experienced and passionate Clinical Lead to join our Community Nursing Team based from our Canterbury locality.


Our Community Nursing Teams provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions.


The service actively works to preventavoidable admissions by providing tailored care that meets the patient need and assists them in actively achieving their long-term health goals and self-care where possible
As the Clinical Lead, you will be responsible for the management and leadership of individual staff members and the quality of the care they deliver. Your team(s) will be aligned to a Primary Care Network (PCN), where you will represent community nursing.


The role requires an experienced Community Nurse to provide sound clinical judgement and act as a role model for the profession. As the senior clinician you will be a decision-maker and a problem solver withaccountability for your team's performance.

Although vaccination against COVID-19 is not mandatory, we are strongly encouraging colleagues in clinical and non-clinical roles who have direct or social contact with patients to be fully vaccinated. Registered health care professional (NMC or HCPC registration) / minimum of five years' experience in health improvement environment. Health related degree and evidence of relevant post graduate development at Level 6. Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: non-medical prescribing, mentorship, coaching, clinical expertise). Management or supervisory qualification / programme of study.


Preparing to Teach in the Lifelong Learning Sector / Diploma in Teaching in the Lifelong Learning Sector, Post Graduate Certificate of Education or Certificate in Education. Experience of developing policy and service development plans with practical experience of change management. Practical experience in managing health related projects/complex caseloads. Experience with using of Microsoft Office.


Evidence of continued relevant professional development and implementation in practice within the last 12 months. Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations. Flexible and adaptable. Valid driving licence and access to a car for work purposes.


Able to support the implementation of local and national agendas for health under the guidance of the senior Practitioner. Ability to prioritise care delivery in a professional manner, taking into account Trust policies and procedures. Kent Community Health NHS Foundation Trust
