You will independently complete assessments and provide support to young people with a range of mental health and wellbeing needs. This includes working within organisational thresholds to ensure appropriate care is delivered.

Develop and implement care and hospital discharge plans, ensuring that care is consistent with changing risks and adapting risk plans accordingly.

Work within established systems, ensuring that discharge planning is completed effectively, and support is offered to the wider team through collaborative working.
*
Experience in working with children and young people with diverse mental health presentations is essential.
*
You will be responsible for signposting to appropriate services and making safeguarding referrals where necessary.
*
Experience working within CAMHS and in child protection roles is required for this role. Full-time
* 44 per hour
