As a Reablement Practitioner you will help to support and improve the lives and wellbeing of the people in Barnsley, helping people regain and develop new skills so that they can return home and maintain their independence You’ll have a life-changing impact.
We are looking for people who want to help people of all ages remain at home and get their life back on track following episodes of illness or an event that has affected their life skills.
Ensure a person-centred holistic approach, which includes activities of daily living and social connectedness, facilitating opportunity to participate in community activity, aiming to reduce potential social isolation and positively influence overall wellbeing.
To act as ‘Duty’ officer on a rota basis by undertaking Triage of new cases and being initial point of contact into service.
UK Driving Licence, access to a vehicle, business insurance
Relevant Level 2/3 qualification
Significant understanding of the social care needs of older people and vulnerable adults
Experience building collaborative working relationships with user’s support network and partner agencies
Enhanced Adult DBS registered to the update service
