An exciting opportunity has arisen for an enthusiastic pharmacist to join a friendly, progressive medicines management service that serves our community in Ealing. The role is to Lead the medicines management service for the Rapid response virtual wards. This supports admissions avoidance and early discharge from acute settings. You will also lead on allocated Community health services that include multiple community services such as MSK, diabetes service, district nursing, special schools, children with learning disabilities, SALT, physiotherapy, dietetics, podiatry and TVN.

Main duties of the job The post holder will be: Leading on developing the medicines management service for Rapid response service virtual ward, supporting the multidisciplinary integrated care team with all medicines related issues, optimising medicines use for all patients under the care of the service Responsible for providing medicines management support and medicines information to community based teams including: MSK, diabetes service, district nursing, special schools, children with learning disabilities, SALT, physiotherapy, dietetics, podiatry and TVN. This list is not exhaustive Responsible for managing and delivering comprehensive prescribing advice and information for clinicians, patients and carers Responsible for ensuring the safe, clinically effective and cost efficient use of medicines To initiate, supervise and undertake medicine audits (where required) on behalf of the pharmacy team Provide appropriate training and education for nursing and medical staff with regards to the use of medicines used in integrated care and other therapeutic area Collate activity and outcome data for respective areas To actively develop, monitor and integrate policies/guidelines and procedures covering all aspects of pharmaceutical care and medicines management within the service The applicant will be required to take part in the out of hours on-call service as well as the weekend morning service on a rota basis. Person Specification Qualifications Essential- Masters Degree in Pharmacy or equivalent
- Registered member of the General Pharmaceutical Council
- Registered member of the General Pharmaceutical Council
- Member of RPS
- Non-Medical Prescriber
- Member of UKCPA or other recognized groups/networks for integrated care
- With relevant and significant pharmacy experience relevant to the post
- Experience in providing a medicines management service or equivalent
- Experience in integrated care (or equivalent) environment Undertaking clinical medication reviews in older people and improving adherence
- Delivering direct pharmaceutical care to patients with long term condition
- Management experience Management skills e.g. recruitment/ appraisal /training/ leadership/ motivation/ sickness absence
- Leading a team of staff to deliver a service
- Experience of MDT audit and/or research & development
- Experience of project management Experience of managing 'change' Working across the wider health and social care integrated/integrated care settings or partnership working within multidisciplinary teams
- Knowledge and understanding of the implications of pharmacy national guidance (e.g. NICE, NHS England, MHRA)
