Job summary

Balham Park Surgery is looking for a dynamic, proactive and forward thinking Pharmacist to join their large multidisciplinary clinical team who are very well established and driven to provide the highest possible service to our patients.

The successful candidate will have experience of working in highly effective teams and can demonstrate positive outcomes in their work output. The individual will need to be autnomous but also able to work to guidelines.

Main duties of the job

The role will involve structured medication reviews and supporting the administrative team in the effective call and recall and support for patients who have complex polypharmacy. Reviewing and implementing practice prescribing policies and protocols, participating in audit, clinical team meetings and whole team training and events.

Overseeing the repeat prescription process and ensuring monitoring is completed for patients in a timely efficient way.

Balham Park Surgery is a large General Practice based in Balham, London.

We have a patient list size of 21000.

Our highly skilled and experienced multidisciplinary team comprises of 5 GP Partners, Management team, Salaried GPs, GP registrars, Pharmacists, Physiotherapists, Social Prescriber, Nurses, HCA, Phlebotomists, Care Navigators and Care Coordinators.

We have a daily whole team get together at midday for coffee/lunch. Regular clinical meetings. We believe we provide a lovely place to work. Wellbeing of our staff is a top priority, with access to an employee assistance service which provides 24/7 support for mental health concerns, stress and anxiety and fresh fruit provided for all staff.

Job responsibilities

The clinical pharmacist in a general practiceorganisation has the following key responsibilities in relation to deliveringhealth services.

These key responsibilities are based on thoseoutlined in Annex B1 of the Network Contract Directed Enhanced Service 2023-24specification

There may be, on occasion, a requirement tocarry out other tasks; this will be dependent upon factors such as workload andstaffing levels.

a.Workas part of a multi-disciplinary team in a patient-facing role to clinicallyassess and treat patients using their expert knowledge of medicines forspecific disease areas

b.Bea prescriber, or completing training to become a prescriber, and work with andalongside the general practice team

c.Beresponsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy, especially the elderly, people in care homes, those with multipleco-morbidities (in particular frailty, COPD and asthma) and people withlearning disabilities or autism (through STOMP Stop Over MedicationProgramme)

d.Providespecialist expertise in the use of medicines whilst helping to address both thepublic health and social care needs of patients at the organisation and to helpin tackling inequalities

e.Provideleadership on person-centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement, whilstcontributing to the Quality and Outcomes Framework and enhanced services

f.Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care

g.Havea leadership role in supporting further integration of general practice withthe wider healthcare teams (including community and hospital pharmacy) to helpimprove patient outcomes, ensure better access to healthcare and help managegeneral practice workload

h.Developrelationships and work closely with other pharmacy professionals across thewider health and social care system

i.Takea central role in the clinical aspects of shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (including mentalhealth and reduction of inappropriate antipsychotic use in people with learningdifficulties), liaison with community pharmacists, and anticoagulation

j.Bepart of a professional clinical network and have access to appropriate clinicalsupervision. Appropriate clinical supervision means:

i.Eachclinical pharmacist must receive a minimum of one supervision session per monthby a senior clinical pharmacist

ii.Thesenior clinical pharmacist must receive a minimum of one supervision sessionevery three months by a GP clinical supervisor

iii.Eachclinical pharmacist will have access to an assigned GP clinical supervisor forsupport and development

k.Toact as the point of contact for all medicine related matters, establishingpositive working relationships

l.Toconsult patients within defined levels of competence and independently prescribeacute and repeat medication

m.Toreceive referrals and directed patients from triage services and otherclinicians

n.Toreceive and resolve medicines queries from patients and other staff

o.Toprovide medication review services for patients in the practice and duringdomiciliary visits to the local nursing home

p.Tomanage a caseload of complex patients

q.Tomanage a therapeutic drug monitoring system and the recall of patients takinghigh risk drugs, , anticoagulants, anticonvulsants and DMARDs, etc.

r.Todeliver long term condition clinics and home visits, particularly for patientswith complicated medication regimes, and prescribe accordingly

s.Toprovide pharmaceutical consultations to patients with long term conditions asan integral part of the multidisciplinary team

t.Toreview medications for newly registered patients

u.Toimprove patient and carer understanding of confidence in and compliance withtheir medication

v.Tomaintain accurate clinical records in conjunction with extant legislation

w.Toencourage cost-effective prescribing throughout the organisation

x.Toimplement and embed a robust repeat prescribing system

y.Toprovide advice and answer medication related queries from patients and staff

z.Toorganise and oversee the organisations medicines optimisation systems,including the repeat prescribing and medication review systems

aa. To improve the quality and effectivenessof prescribing through clinical audit and education, to improve performanceagainst NICE standards and clinical and prescribing guidance.

bb. To develop yourself and the role throughparticipation in clinical supervision, training and service redesign activities

cc. To ensure appropriate supervision ofsafe storage, rotation and disposal of vaccines and drugs. To apply infection-controlmeasures within the practice according to local and national guidelines

dd. To provide subject matter expertise onmedication monitoring, implementing and embedding a system

ee. To support clinicians with themanagement of patients suffering from drug and alcohol dependencies

ff.Toactively signpost patients to the correct healthcare professional

gg. To manage a caseload of complex patientsand potential care institutions and to provide advice for the GP management ofmore complex patients or areas such as addictive behaviours, severe mentalillness or end of life care

hh. To review the latest guidance, ensuringthe organisation conforms to NICE, CQC etc.

ii.Toprovide targeted support and proactive reviews for vulnerable, complex patientsand those at risk of admission and re-admission to secondary care

jj.Tohandle prescription queries and requests directly

kk. To provide proactive leadership onmedicines and prescribing systems to the organisation, patients and theircarers

ll.Tosupport in the delivery of enhanced services and other service requirements onbehalf of the organisation

mm.Toparticipate in the management of patient complaints when requested to do so,and participate in the identification of any necessary learning brought aboutthrough clinical incidents and near-miss events

nn. To undertake all mandatory training andinduction programmes

oo. To contribute to and embrace thespectrum of clinical governance

pp. To attend a formal appraisal with yourmanager at least every 12 months. Once a performance/training objective hasbeen set, progress will be reviewed on a regular basis so that new objectivescan be agreed

qq. To contribute to public health campaigns(, COVID-19 or flu clinics) through advice or direct care

rr.Tomaintain a clean, tidy, effective working area at all times.

In addition to the primary responsibilities,the clinical pharmacist may be requested to:

a.Supportdelivery of QOF, incentive schemes, QIPP and other quality or costeffectiveness initiatives

b.Agreeand review prescribing formularies and protocols, and monitor compliance levels

c.Improvethe data quality of medicines records and linking to conditions

d.Delivertraining, mentoring and guidance to other clinicians and staff on medicineissues

e.Workingin partnership with pharmacists and clinicians in linked hospital(s), improvethe safety and quality of prescribing after discharge from hospital admissionsand attendance

f.Provideleadership and support to prescription administrative/dispensary staff

g.Producepharmacy/prescribing newsletters or bulletins on a quarterly basis

h.Developa specialist area of interest

i.Workwith community pharmacists, hospital pharmacists and other stakeholders in themedicines supply chain to improve patient experience and manage incidents

j.Supportvirtual and remote models of consultation and support, includinge-consultations, remote medication review, and telehealth and telemedicine

k.Undertakeany tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner
