Position Overview: We are seeking a dedicated Transition Teacher to join our team. The Transition Teacher will play a pivotal role in facilitating a smooth transition for students moving between different phases of their educational journey, ensuring continuity of learning and personalized support.

Key Responsibilities:

  • Develop and implement transition plans for students entering or exiting our institution, collaborating with teachers, parents, and support staff to address individual needs.
  • Provide guidance and support to students in navigating academic, social, and emotional challenges during periods of transition.
  • Foster a positive and inclusive classroom environment conducive to successful adaptation and integration.
  • Monitor students' progress and provide timely feedback to facilitate their transition journey.
  • Collaborate with colleagues to align curriculum and teaching strategies across different educational levels, promoting consistency and coherence.
  • Stay informed about best practices in transition education and contribute to the continuous improvement of transition programs.

Qualifications:

  • Bachelor's degree in Education or related field (Master's degree preferred).
  • Valid teaching certification/license.
  • Proven experience in transition education or related field.
  • Strong understanding of child development and educational psychology.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate effectively with diverse stakeholders.
  • Commitment to fostering an inclusive and supportive learning environment.
  • Flexibility, adaptability, and resilience in navigating change.
