To work as part of a multi-disciplinary team, providing nursing and rehabilitation for rehabilitation beds.? To undertake nursing assessments without supervision and to determine patient need/risk and plan, implement and evaluate their care.? In the absence of senior colleagues take charge of the unit, manage the admission of patients, provide care and discharge planning whilst contributing to the effective working of the Rehabilitation Unit.? To work in partnership with multi-disciplinary members of the team, other professionals and agencies, to meet the health and rehabilitation needs of patients within the Rehabilitation Unit.? To maintain standards of clinical nursing care using an evidence-based approach within the Unit, incorporating principles and values of clinical governance.? We providecommunity health services to more than two million people across eleven London boroughs and Hertfordshire.

To identify, prioritise and implement programmes of care that meets the needs of the clients/patients using an appropriate nursing model. To undertake activities that promote health, reduce risk and disease prevention. To provide care and attention to the needs of vulnerable adults including risk assessments and taking appropriate action as required. Take a pro-active approach towards discharge planning according to local policy. To develop and utilise a range of nursing skills such as continence assessment, ECG recordings and venepuncture, IV drug administration, wound assessment and management. NMC Registration

Evidence of other post-registration education and training


Recent clinical nursing experience negotiating and liaising within a multidisciplinary team
Working with people with complex health needs
Experience of undertaking general assessment by utilising evidence based care.
Previous experience in the Nursing process, care planning and fluent in record keeping
Experience of providing holistic care that involves the multidisciplinary team

Experience of training other students
Participation in audit research projects service development
Post registration experience in a NHS community environment

Knowledge of out of hospital care and the role of the Community Nurse
Awareness of the management and use of data in the NHS setting
Knowledge of risks involved in working in the community.
Knowledge of management of long term condition e.g. Diabetes and an understanding of the concept of self- management.
Knowledge of general wound care
Promotion of continence
Knowledge of recent government legislation and changes in community care delivery
Ability to communicate well verbally and in writing such as computerised documentation and data entry
