An exciting opportunity has arisen for a full time Band 7 Clinical Specialist Occupational Therapist or Physiotherapist within the integrated discharge team team at the Royal Sussex County Hospital. The team provides rapid and comprehensive assessment to patients in the A& E Department, Acute Admissions Unit (AAU) and two short stay wards.


The Integrated Discharge Team consists of Occupational therapists, Physiotherapists, Discharge Coordinators and Rehab Assistants. The team works within the integrated model creating a seamless service across Physiotherapy and Occupational Therapy, enabling an efficient journey for the patients.


The successful candidate should be pro-active, flexible and demonstrate excellent communication, clinical and organisational skills required to lead and manage in a fast paced environment, working on providing excellent assessment, discharge planning and placing a key focus on admission avoidance.


The Clinical Specialist therapist is responsible for providing clinical expertise, leadership, support and supervision within the Integrated Discharge Team (IDT) based on Level 5 at the Royal Sussex County Hospital.


To work across A& E, Acute Assessment Unit (AAU), Short Stay Wards, C5 and the rest of level 5 as required.


To play an active role in the further implementation and rolling out of a front door seven-day service model.


As a Clinical Specialist Therapist, you are required to provide a wide range of highly specialist Occupational Therapy/ Physiotherapy interventions as part of an evidence based, flexible and patient-centred approach with a specific aim of avoiding unnecessary admissions and facilitating timely discharges for patients with complex health, mental health and social care needs.


To participate in a 7-day rota including bank holiday and weekend cover.


To participate in and facilitate the planning, development, and evaluation of Therapy services within the Integrated Discharge Team, holding responsibility for defined projects.

Fieldwork supervisor / placement practice educator training
Extensive postgraduate experience as an occupational therapist/ Physiotherapist.
Experience of working in a range of clinical specialties e.g.: acute medicine cardiac respiratory medicine palliative care/end of life dementia care
Experience working as part of a multi-disciplinary team in an acute hospital
Training in leadership and management
Experience of leading a project or service development

Professional skills to inform suitability for discharge from hospital care and associated risks
Understand and implement appropriate standards, including risk assessment, in all work undertaken
Presentation and Training skills


Ability to receive and impart sensitive information related to disability and loss of function
Ability to reflect on and critically appraise own performance
