Key Responsibilities: Carry out occupational therapy assessments vulnerable and disables service users in thir own home for the required equipment and minor adaptations to maximise their independence. Supporting with triage and decisions around suitability with OT, reablement, etc. Moving and Handling assessments, including providing equipment, identifying reduction of care where suitable. Working with the existing D2A OT to cover hospital discharge.Requirements: Be a qualified Occupational Therapist registered with the HCPC. Experience of working with people with physical disabilities.
