Full Time post working between South Shore Primary Care Centre, Fleetwood Hospital and home working (Monday -Friday). Part time hours will be considered.

The service is a community based service providing coordinated and integrated care for patients over the age of 18 with ongoing symptoms post contracting Covid.

Referrals are accepted from all Primary and Secondary care services.

Main duties of the job

You will undertake a generic role as a Clinical Care Coordinator learning skills and knowledge from other professionals to provide an integrated model of care to patients aiming to ensure their care is provided in a coordinated way but also supporting them to manage their own care better. We are looking for the successful candidate to be able to:o Assess, plan implement and evaluate care. o Ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team. o Work in partnership with the wider community and primary care teams to provide holistic patient care. o Be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals.

About us

The team is multidisciplinary comprising of GP's, Nurses, Occupational Therapists, Physiotherapists, Dietitians, Clinical Psychologists and support staff.

The service operates across the whole of the Fylde coast, and based at South Shore Primary Care Centre and Fleetwood Hospital.

Job responsibilities

Working with primary, secondary and community colleagues, you will:

- Be an autonomous practitioner who holds delegated responsibility for a defined caseload, coordinating care, undertaking specialist assessments and planning safe discharges for patients within the Long Covid Service.

- Assess patients in their own home or clinic setting: plans, implement and review care of patients with complex on-going health and social care needs.

- Carry out specialist assessment and treatment including the prescription of essential equipment as part of a package of rehabilitation. Specifically assessing issues related to the barriers which prevent the patient carrying out activities of daily living e.g. mobility, moving and handling, functional restrictions, activity tolerance, hand eye coordination, positioning, cognitive impairment, environmental restrictions etc.

- ensure that patients receive the care they need linking into other services and agencies (statutory and voluntary) as appropriate and ensure the patients understand their medical conditions and medications they are taking.

- create outcome goals for the patient to achieve during their time within the CFS and lead on the development, implementation and monitoring of the patient specific care plan to ensure these goals are achieved.

- Work in the community as a lone practitioner in line with the Trusts Lone Worker Policy.

Person Specification

EDUCATION/ QUALIFICATIONS

Essential


  • Registered Nurse
  • Degree or Diploma in professional area of practice
  • Current registration with NMC
  • Postregistration training in Frailty and/or Long Term Conditions.
  • Three years post qualified

Desirable


  • Specialist Practitioner degree.
  • Non-Medical prescribing qualification or working towards this
  • ENB 998 Teaching and Assessing in clinical practice or equivalent.

EXPERIENCE

Essential


  • Experience of care co-ordination of people with complex health and social care needs.
  • Experience of complex discharge planning
  • Experience of delivery of complex care in or out of hospital setting
  • Experience of caseload management.
  • Experience of working within a multi-disciplinary team within a care setting
  • Experience of supervising junior or support staff
  • Experience of managing Long Term Conditions

Desirable


  • Experience of clinical audit.
  • Experience of research and development
  • Experience of working within the community setting.
