Adecco are supporting a London Borough Council team in a service role as a Leaving Care Personal Advisor We are currently seeking a dedicated and compassionate Personal Advisor for our client in the Leaving Care Service. This temporary position offers an excellent hourly rate ranging from £20.29 to £25. PAYE or Umbrella (Circa £45,5 K per annum). Start date of June 2024. 35 hours per week. Located near Canary Wharf. As a Personal Advisor, you will play a crucial role in supporting and guiding young people as they transition from care to independence. You'll play a crucial role in supporting and guiding young people as they transition from care to independence. If you have experience in conducting assessments, developing care plans, and a commitment to promoting diversity, this could be the perfect role for you. Join our client's team and help shape a brighter future for young people in the Leaving Care programme Responsibilities: Manage a caseload, developing and reviewing Pathway Plans in collaboration with service users. Respond to requests for support and determine the level and type of assessment required. Complete Pathway Assessments alongside young people, carers, and social workers. Ensure Pathway Assessments and Plans are holistic, addressing all relevant user categories. Liaise with specialists and agencies, involving them in the assessment process. Support users and carers in participating fully in Pathway Planning. Assess social care and accommodation needs, considering individuals' wishes and family dynamics. Offer advice and support in areas such as health, education, training, employment, and personal relationships. Review Pathway Plans at regular intervals to ensure they meet the individuals' needs. Maintain accurate and up-to-date records, complying with policies and procedures. Identify service gaps and provide recommendations to management. Requirements: Knowledge of child care legislation, including the Children Act 1989 and associated guidelines. Familiarity with the challenges faced by young people leaving care and the services available to support them. Experience conducting assessments and developing care plans. Strong communication skills, both verbal and written. A commitment to equal opportunities and promoting diversity. IT literate and willing to undertake training as needed. Flexibility to work outside of regular office hours, including evenings and weekends. A satisfactory enhanced DBS disclosure.
