Lead Occupational Therapist Falls Prevention and Bone Health Service

We are looking for a talented and dynamic clinical lead Occupational Therapist for our new Hounslow Falls and Bone Health Service.

You will provide clinical leadership and support day to day management of the service and be responsible for developing clinical pathways in and out of the falls service to improve the patient experience and reduce the number of people who fall. You will also support the Service Manager with mobilisation, delivery and evaluation of the service, recruitment, clinical delivery, staff support and development.

You will work as part of a multi-disciplinary team, including physiotherapy, dietetics and nursing, with patients and their carers to offer a multi factorial falls risk assessment followed by a range of multi-disciplinary interventions. You will also work closely with colleagues in the acute Falls / Frailty clinic and the local authority home hazard service. You will provide support, supervision and training for junior and support staff, and students.

Main duties of the job

Be responsible for day to day operational management, planning, co-ordinating, delivering and evaluating the service.

Contribute to business planning and service development

Champion best practice and the importance of occupational performance as part of falls prevention, addressing all performance components within assessment and intervention.

Implement interventions which facilitate and sustain change within daily performance, whist considering values, motivation, pattern of occupation, choice making and meaningful occupations in a way which compliments physical and physiological falls prevention interventions.

Provide holistic, multifactorial falls risk assessments and comprehensive fracture risk assessments and bone health management for clients who are at risk of, or worried about falling.

Triage referrals received from GPs, other health professionals and others, being aware of quality and appropriateness of referrals, and deal with these as necessary.

Independently manage a caseload without direct supervision and to be professionally and legally accountable for all aspects of own work, including the management of clinical risk.

Participate in pathway design and promotion of the service to encourage new sources and routes of referral to optimise the scope and impact of the service.

Comply with the Royal College of Occupational Therapy Core Standards of Practice and HCPC Standards and all other relevant national and local policies and procedures.

  • Specialist knowledge gained through post graduate courses

  • Working in the NHS at a specialist or advanced level, in the field of falls prevention.
  • Evidence of providing professional/clinical leadership
  • Clinical education of less experienced staff, students or assistants
  • A good understanding of clinical governance including quality, audit and risk management and its relevance to this clinical area
  • Cross organisational / MDT working
