Role Description

This is a full-time Care Coordinator role at Mountbatton Care Services located in Bootle, with flexibility for some remote work. The Care Coordinator will be responsible for coordinating and supporting the delivery of care to clients in the Bootle area. This will involve liaising with clients, families, healthcare professionals, and other stakeholders to ensure that their needs are met at all times. The Care Coordinator will also be responsible for managing staff schedules and ensuring that quality standards are maintained across the service.

Qualifications

  • Bachelor's degree in Nursing, Social Work, or a related field
  • Experience in a care coordination or management role
  • Excellent communication, organizational, and problem-solving skills
  • Ability to work collaboratively with clients, families, healthcare professionals, and other stakeholders
  • Experience with electronic record keeping systems, such as CareNotes or SystmOne, is a plus
  • Familiarity with the local healthcare system and services is desirable
  • Full UK driving license and access to a vehicle is essential
