MHSOP have exciting plans to modernise our inpatient workforce to meet current and future challenges, This role will be an additional ACP role for MHSOP inpatient services (the other based at Highbury hospital)

This position will be based at Millbrook Mental Health Unit, in Sutton in Ashfield. The roles focus will be the physical and mental health care of older adults with Dementia and/or Mental health issues that have required them to be admitted to inpatient services.

MHSOP are dedicated to the development of the ACP role within services and are leading the way in terms of the specialism of Mental Health in Older Adults and the holistic imapct of ACP intervention.

It is anticipated that this role will develop over time in response to changes in service delivery and therefore we need clinicians who are energetic, eager to develop and are forward thinking with the potential to adapt to change and meet new challenges.

Main duties of the job

The Advanced Clinical Practitioner uses their acquired expert knowledge base, complex decision-making skills and competences for expanded scope of advanced clinical practice. The specific characteristics of the role are shaped by the clinical context that the individual practices. They are part of the multi-professional team.

A fully qualified ACP must be able to:

  • Complete a full system and physical or psychological/psychiatric exam
  • Take a systemic and/or psychosocial patient / prisoner or service user history
  • Make diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results
  • Initiate, prescribe and monitor the impact and outcome of treatments and medication including efficacy, side effects, contraindications and risk
  • Devise an individual clinical management plan that includes all aspects of multi-disciplinary care
  • Screen patients for early signs of disease and risk factors and relapse indicators
  • Carry out where appropriate, invasive and non-invasive diagnostic and therapeutic procedures with due regard to mental capacity and advance decisions
  • Refer to other health professionals as appropriate
  • Discharge from hospital or services as required
  • Run independent out-patient clinics seeing new and review individuals as required

Job description

Job responsibilities

The post holder will be based in services connected to Nottinghamshire Healthcare NHS Foundation Trust within an agreed specialty / service area.The Advanced Clinical Practitioner (ACP) will share leadership with other senior clinicians and managers using specialist knowledge and skills to provide healthcare autonomously to patients, prisoners or service users in their specialty. The ACP is accountable for independent clinical assessment, diagnosis and treatment of these individuals with undiagnosed and undifferentiated conditions. In addition to this the ACP is also responsible for appropriately referring individuals receiving care to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe discharge as required.The post holder will be a registered nurse or AHP and hold an MSc in Advanced Clinical Practice or equivalent at MSc level. They will be an independent Non-Medical Prescriber registered with the NMC or HCPC. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of medicine, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.ACPs are part of the multi-professional team and may contribute to medical rotas, the work pattern of each individual being determined by both their level of experience and competence. There will be opportunity as experience is gained through progression.The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients / prisoners and service users presenting to their specialty by providing clinical advice and support to nursing staff and other health care professionals. They will provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service.The role will include developing audit and research projects and participating in up-dating and implementing departmental policies, protocols and guidelines, in line with national guidance and contemporary evidence.The ACP will have protected time to maintain and further develop their professional competence.They will have annual appraisal from medical and nursing / AHP supervisors. The ACP will also provide ongoing supervision and support, participating in the induction and training of nursing and medical staff. As a senior member of the team, the ACP will also play a pivotal role in the operational development of their specialty.They will initiate, manage and drive change within the department, innovating changes in practice for the benefit of patient care in line with current trust and local programmes. Approximately 20% of the post holders time will be used for these purposes.The ACP will undertake the role in accordance with Trust and Departmental guidelines but have freedom to act within broad policies and protocols, in accordance with professional responsibilities and boundaries.

Essential


  • Current registration with the NMC or HCPC
  • Clinical based Post-graduate Degree
  • MSc Advanced Practice
  • Non-Medical Prescriber (V300)

Desirable


  • Management/Leadership Qualification or attendance at equivalent Leadership Study Days (Masters qualification or equivalent)
  • Recognised teaching or mentorship qualification at degree level or equivalent

  • Evidence of working at a senior position within Health Care of Older Peoples services.
  • Demonstrate ability to work across all 4 pillars of 'Advanced Clinical Practice'
  • Experience of working with Older People with Mental Health issues & Complex Physical Comorbidities.
  • Evidence of extensive expert professional/clinical knowledge managing unwell patients in the community, primary or acute setting.
  • Specialist clinical training and CPD in relation to patient history taking, clinical examination and developing management plans for patients.
  • Knowledge of national and local NHS healthcare agenda, standards, research and its implications.
  • Participation in research, audit and evaluation
  • Knowledge and experience of chronic disease management and long-term conditions.
  • Evidence of managing staff and multidisciplinary teams
  • Evidence of relevant involvement in meeting the Trust clinical governance objectives
