Our integrated teams of Nursing and Allied Health Professionals deliver care to people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in the patient’s own home, including care homes, enabling them to be independent, self-caring and live their optimum lives, reducing hospital admissions.

Services range fromcommunity nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry.

Supporting the efficient running of these services are administrative and managerial roles. All roles, at all levels, are vital in transforming community health services, ensuring high quality care is provided both now and into the future based around the needs of our patients - together we all make a difference.

There is an exciting opportunity for a motivated and enthusiastic qualified nurse to join our Healthy Families Team in Mansfield.

The Healthy Families Team deliver Universal and targeted health services to the 0-19 year population in Nottinghamshire. The post holder will be responsible for supporting the day to day management of a defined Caseload, working within a skilled and mixed team.

This is an excellent opportunity for a suitably qualified, experienced nurse with sound communication skills to develop their role within the community setting. We offer a friendly and supportive working environment with excellent development opportunities.

As part of the Healthy Families Team, the post holder will be responsible for the delivery of public health and universal and targeted interventions across the 0-19 years age range; This will be delegated to them by the Specialist Public Health Practitioner for a defined population. This will be delivered in line with the National and Local guidelines and public health priorities.

You will be responsible for supporting and clinically supervising the skill mix team allocated to a defined geographical population to deliver and maintain high quality professional standards of practice.

The team will work proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of competencies required for the post.

You will work with children, young people and families within the home environment, school or other identified community setting.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust.
