The Role We are seeking a Patient Safety Investigator to join our Clinical Governance team. This is a critical role that supports our Patient Safety Incident Response Framework and strengthens our culture of openness, accountability and learning. You will lead and contribute to patient safety investigations, ensuring that reviews are structured, impartial and aligned with national patient safety requirements. The post holder will work with clinical and non clinical colleagues across the organisation to analyse incidents, evaluate evidence and produce high quality reports that identify learning and improvements. You will also support engagement with patients, regulators and insurers, ensuring that The London Clinic continues to uphold a high standard of transparency and assurance. This role requires a current professional registration with a UK regulatory body and prior experience or clear knowledge of clinical governance, the patient safety framework, PSIRF methodology, Datix processes and the wider principles that underpin patient safety and quality systems . Candidates without a professional registration or without an understanding of these frameworks will not be suitable for this position. Key Duties: Lead and support patient safety investigations that are timely, proportionate and aligned with PSIRF requirements. Use systems-based approaches and human factors principles to understand contributory factors and develop actionable learning. Gather, review and analyse evidence including clinical records, statements and interviews to ensure accuracy and clarity. Produce structured investigation reports that provide assurance and support organisational learning. Analyse Datix data to identify themes, trends and areas for improvement. Support the monitoring and review of improvement actions with clinical teams. Contribute to patient safety training and awareness sessions across the organisation. Engage with patients, families, regulators and insurers with professionalism and sensitivity. Ideal Candidate This role suits a registered clinician who is ready to transition from direct clinical practice into a patient safety and governance focused career. You will have strong clinical judgement, an analytical mindset and a firm understanding of the principles that underpin safe care. You will be comfortable examining complex situations, asking difficult questions and working with colleagues across all levels to support learning and improvement. Professionalism, attention to detail and sound communication skills are essential. Skills and Experience: Current registration with a UK professional body such as NMC, HCPC or GMC. Clear knowledge of patient safety principles, the national patient safety framework and the fundamentals of clinical governance. Understanding of PSIRF and ideally experience supporting or delivering PSII investigations. Awareness of Datix processes and the use of incident reporting systems. Experience in patient safety, quality improvement or clinical governance within an acute healthcare setting. Relevant clinical experience in a hospital environment. Experience conducting or supporting investigations into patient safety incidents. Strong analytical skills and the ability to interpret data and identify patterns. Skilled in producing clear and concise reports for assurance purposes. Confident communicator with the ability to engage with patients and staff sensitively and professionally.
